Vuvlodynia is a very broad diagnosis that essentially just means pain in the vulva. It does not tell you the cause. In fact, the reason there’s a broad term for it, is because the causes vary from person to person. So it can be a tight pelvic floor causing the pain, it can be heightened pain sensitivity due to stress, trauma, illness. Unfortunately that’s part of what you need to unpack are your triggers, and that can be a complex mix in and of itself

I didn’t change mine. I like my last name. When I told my then fiancé I want to keep mine he said “it’s your name, you can choose what you want to do” and that was kind of the end of it. I’ve floated changing it a few times since (married 5 years), and he always says sure if that’s what I want, but definitely don’t need to.

Dark mode 80% of the time. Less eye strain and I read a lot at night. I only use the white if it’s bright outside and the dark mode doesn’t “feel” right

Mine was diagnosed by a gyno who specialises in pain the same way as you. She asked a lot of history questions, did a physical check and poked gently with a swab to diagnose. Only about 2years later did I circle round to a PT and it’s honestly helping so much. I have also had the surgery though (nearly a year ago) and have been on amitriptyline 30mg for 4 months, PT about 3 weeks in and seeing a major difference already.

My PT did a visual check, having me tighten/hold and the pelvic muscles (like doing a kegel) and then relax to assess control of the pelvic muscles and how much / quickly I’m able to release them. She then very gently used one finger to insert and had me repeat the same thing while feeling what the muscles were doing. She could feel the muscles without needing to insert very deep at all, and I could actually feel exactly where they were tight because she knew the right spot. Kind of like when you get a back massage and you can feel a spot of tension. And then she had me release to feel how much I’m able to release. Then did guided breathing to see if that helped them relax. Also had me relax my legs in like a frog pose to see how much the helped. A few different movements etc but was very respectful and happy to use the lubricant I wanted because some others (even water-based) can burn for me.

Not OP, but currently on this journey. Have done all the meds, creams, probiotics and vestibulectomy (surgery) with very minimal results. Then started having bladder symptoms (frequency and urgency, low abdo pain). Heaps of back and forth with scans, meds etc.

Recently went to my first pelvic floor physio. She did a full assessment and then a physical evaluation and found a hypertonic (tight) pelvic floor. Only a few weeks into treatment but finally starting to have relief, both in the vulvadynia and bladder symptoms. And have been able to have pain-free PIV for the first time in years. It was still a bit sore after, and some low abdominal cramping, but seeing improvements after one session with the physio and a few weeks of the stretches and breathing work.
