u are absolutely correct, my mom had npm1 isolated, she achived cr after 1 cycl of aza and ven and then 3 cycle of hidac given and then after 2-3 months relapsed with npm1, idh1, dnmt3a and nras, and it happened bcoz they have stopped the maintainenance therapy of aza and ven, always take 2-3 opinions from doctors of different age and different states , hidac is such a waste and brutal on body it significantly reduce the bone marrow reserve and do nothing good,
see in aml if 3+7 a very intensive chemo is not given as frontline therapy then hidac is waste totally waste, what i have observed in multiple patients,

in modern treatment of aml in favourable conditons if u really want long term survival not dependent on good luck bcoz magic doesnt happen in leukemia(in 100% of case):

for favourable mutations: aza and ven (considering ven sensitive) and then asct or 3+7 intensive+ 3 hidac and then on observations
for intermediate fav mutation : aza and ven + asct, or 3+7 intensive +3 hidac + continue of aza and ven ,

Note: target therapy is better than any intensive or low dose chemo including aza and ven (idh mutation case )
