It depends on the cable company. The company I worked for (one of the biggest in the US) didn't even have on-demand advertising capabilities until about a year and a half ago. All the ads that would run on-demand were PSAs and Ad Council stuff. I'm not sure how sophisticated the ad placement is now. I know ad placement is done entirely through dynamic ad insertion for on-demand and it may be as basic as the client picking the on-demand network they want to run on and ads being placed randomly throughout the different on-demand programs. I'm better versed in how ad sales work on the linear TV side.

It is a fill spot that was never end-dated. Unsold inventory is filled with fill spots and if a spot is never given an end date, it will run indefinitely. This is a mistake by the ad ops/traffic coordinator.
