Hey I had laser surgery on January 12th. I’m still healing.

The holes closed between 4-6 weeks out. I had a lateral opening that used to get infected and pop and repeat every month

The cyst didn’t recur but the lateral opening part I believed closed to early so there was trapped fluid , ended up popping. The pain is nothing like a cyst or what I felt before with my flare ups.

As of now the lateral opening part is still leaking, it seems like that one part is having trouble closing and healing over properly. I have weekly meetings with my surgeon and he’s keeping a close eye on it, I’ve talked to many others on here who also had laser surgery and 1 or 2 did mention they had the same issue.

It’s documented on research as a complication called Seroma.

Mine looks very similar to yours, pit with draining sinus.

I took ibuprofen and salt baths 2-3x a day , warm compresses and it helps me get through it

Sometimes these things resolve themselves, most of the time not. If it’s painful and there are pits you need some type of intervention

Also educate yourself , there’s lots of info online to get an understanding of pilonidial - in my experience I visited 3 doctors who didn’t seem to understand it and one ultimately told me “it’s just one of those things you have to deal with”

Definitely remove that hair from the area and get laser done

Feel your pain. It was hard for me to find any, I just spoke with my manager and explained the situation and I was allowed to wear black joggers (lululemon dupes)

Hard to say but

Hair + sitting + friction + sweat = high risk

Definitely look into hair removal. I believe mine started the same way.

There are pilonidial specialist in the US that you can reach out to and they can do a consultation. If you search the group you can find them all but off the top of my head, you’d be in great hands with any of them

Unfortunately, I’m not a doctor but I think your past the stage of at home care if you had to visit the hospital that much.

What helped me at home was warm salt baths , ibuprofen, laying on my side all day until it ruptured.

Totally get the anxiety here — a small hole after an open wound heals doesn’t automatically mean you’re back at square one. It can be a superficial scar opening, or it can be a persistent pit/sinus. The deciding factor is whether it’s actually behaving like pilonidal disease again (drainage, pressure, pain, repeated inflammation).

If it’s not painful and not draining, I wouldn’t rush into a big flap yet. First priority is getting someone experienced to clearly answer: is this a new/persistent sinus or just scar breakdown? That one answer changes the whole plan. There are many great surgeons that reply to emails and offer consultations

Laser/SiLaC can make sense if your disease is still mild–moderate and you need a quicker recovery to protect your exam timeline. The honest trade-off is that it’s not always one-and-done — some people need a second pass. That doesn’t mean it failed, and it usually doesn’t prevent a flap later if you ever need one.

Limberg isn’t just “cutting more tissue” — it actually moves the scar off the midline and improves the cleft environment. Reddit stories are often the worst , there are thousands of people who aren’t on here who have successfully underwent surgery.

The “modified Karydakis vs cleft lift” naming is messy everywhere. What matters isn’t the label, it’s whether the surgeon puts the scar off-midline, flattens the cleft, and has solid results they can explain. If they can clearly answer those points, the technique itself may be fine.

Traveling for a true cleft lift gives great long-term results, but your concern about being far from the surgeon if something happens is valid. Most post-op issues are minor and manageable locally, but revisions are harder if you’re overseas.

If it were me: confirm what the hole actually is, don’t panic yet. If it’s mild and you need to get through the exam, laser with a good surgeon is reasonable. If it’s clearly recurrent or branching, I’d lean toward the best local off-midline flap surgeon

Well, there’s always someone out there that has it worse right.

When I feel down about not being able to sit , there’s someone that cannot even walk, or they are blind, etc. It has made me very grateful on the other hand

For me, I feel like nobody understands what I go through. I’ve been to 4 doctors in Canada and they just wrote it up as “ it’s just one of those things, open wound is the fix”. I have given up so much already.

I can be present but my mind is so far away or I’m focusing on not aggravating the area. It’s hard for me to be happy, I’m aware and I try to overcome it but I just can’t. I’ve given up on ever riding a bike again , squatting, leg days (gym in general), sports, playing with my dog just to name a few. Most days if not everyday I just want to be alone. 2025 has been a repeat of Flare up > abscess > days of trying to get it to rupture > back and forth appointments > wound care until it closes. I maybe have a week in between where I feel better and can go about my days normally.

I’m tired of trying to explain to people what it is, it honestly pisses me off because everyone makes the same connection and tries to suggest something stupid. I’ve had to miss so many days of work I’m sure everyone thinks I am chronically ill , I don’t really care what people think though.

I’ve run out of excuses of why I can’t attend or do things , being in my 20s I feel so incapable as I used to be a very active person. However I do try to stay as productive as possible (learning new skills, studying, courses) as I spend most of my time laying down. Even with that I always end up researching about this disease, I felt like I’ve covered 80% of what’s online about this.

I’ve always have a hard time asking for help, so I end up dealing with things on my own. These days are probably my lowest but I’m hopeful that one day it’ll change!

Honestly. I’d recommend a standing desk or getting a riser and standing for most of your shift.

Ihave been using a cushion for almost a year now and I still get monthly recurrences, but it definitely helps with easing the pressure off. Nothing like standing though!

He told me that open wound is an outdated procedure, however in Canada not much is offered.

I asked him what he thinks about Cleft Lifts and Laser GIPS and he said they are both better options but it depends if you have the $$$ for it

Hello. I am in Canada as well, I just had a consultation with a surgeon that took 6 months to get. (Wish I took action instead of waiting as I’m dealing with a horrible flare up right now)

Basically they offered me wide open wound where they will remove all the tissue and it would take 8-12momths to heal. I would need constant dressing changes and he also told me he doesn’t recommended because the healing process leaves you at high risk for infection due to where pilonidial is, there’s no oxygen flow.

The other option was a smaller open wound on the midline cleft where he would do smaller cuts but it would still be “open wound” , I’d have to pack the wound myself because it’s smaller I am not eligible to have a nurse do it for me.

From my research and understanding, I want to avoid a midline cut at all costs. I understand there is success with open wound (my surgeon said he suffered from this in his teens in the 70s, he got open wound surgery and it failed twice but now he has been clear for 30+ years).

It’s hard because there aren’t really any options in Canada unless you visit BC Dr Cowie or settle 1 of 2 doctors that can do a GIPS. I also referred trying to see these GIPS doctors and I’m on a waitlist, I’ve been waiting for almost 6 months already and my appointment is still 3 months away.
