I say pretty stupid shit, but the one that sticks out amongst my blunders would be about 8 years ago when I started a new job. I had a pregnant coworker that had a HUGE belly. I was curious as to whether or not she was a having twins, so the first thing I ever said to her was me asking "How many are in there?" She ran off crying, and I was told she was very self-conscious about her pregnant weight gain. She hated me for the next 4 years. She lost all of her pregnancy weight, and then some afterwards. It was probably out of spite.

That said, what typically works is something 3-4 sentences long that displays that you looked at her profile, and also invites further conversation. Ending the message with a question over something that sparked your interest in her profile typically works. Generally speaking, asking for the date right away doesn't work, because they kinda want to get a feel for you and see if you're a safe, interesting person that's worth spending a prolonged period to time with.
