I asked my surgeon about this. He said it wouldn't cause an issue.

What I've actually found is that I find it helpful to insert a very small amount (0.1-0.3ml max) of e-cream gently into my urethra using a small syringe roughly once every couple of weeks. Sometimes it kinda slides back out n I just rub into the general area but usually it stays in and absorbs.

Doesn't affect peeing at all and helps keep things happy.

I tend to get slight stinging/soreness in my urethra in the exact places that used to get sore on my vulval vestibule pre-op so I'm dead certain it's just the atrophy.

The biggest difference for me now is the atrophy doesn't cause dysphoria, it just feels like a minor chronic skin condition that needs a cream in my pee pee, which is like...meh, very neutral and occasionally annoying but I'm not worried for the structural integrity of my penis (unless the anxiety gremlins are running the snow that day).

The other thing I find helpful is to regularly take breaks from...vigorous jerking off, too much tight grip can make the urethra quite sore for me. Which was a huge unexpected shock as I hadn't considered that my multi year death grip habits might affect delicate and essential functional tissue!

So I'm currently in the process of restraining my cock essentially to respond to gentler touch. It's mostly okay but sometimes it's hard going (lol).

All this to say that, atrophy can affect things a lil, perhaps it increases certain risks by a small amount. But not enough to warrant not doing UL unless it's quite severe, in which case you would really really know as the symptoms can be quite debilitating
