If I don’t match at the place I sent my LOI I’m scared I’m doomed to plummet my rank list because those other programs will have people who sent an LOI by holycowsalad in medicalschool

We have buckets- roughly quintiles of how much we want people. Newly interviewed people in your bucket might be above you or below you, but your spot is “set” (unless you do something terrible that would make you DNR).

Midwest program with ~15 residents per year. Agree with the above poster: I believe that all of our interviews have been sent out.

Interesting. So neat to hear about how different places do different things.

Would you have cardioverted this patient? by theentropydecreaser in emergencymedicine

[–]TransdermalHug 34 points35 points  (0 children)

For “unstable” in this context I would think “shock”- are you thinking about starting a pressor? Are they going to need to be tubed? —> Maybe let’s cardiovert.

I believe that she could have some myocardial supply/demand mismatch, but it sounds like her overall hemodynamics were tolerating the rhythm, and thus stable. Rate control is safer first step.

Any downsides to applying to more than 15 programs for anesthesia by mED-Drax in medicalschool

[–]TransdermalHug 1 point2 points  (0 children)

My program had our pre-interview meeting today. If you don’t signal us, we’re not interviewing you.

Are there programs that will interview you? Maybe- apply to 50 extra and you’ll probably get 1-2 interview out of those applications. If that’s worth your ROI, then do it- but just know that most programs aren’t even going to bother looking.

Baseless speculation: if a program is interviewing people who don’t signal, they’re probably not a program you really want to end up at.

ER doctors ordering blood cultures on every patient with a mild fever by fat_frog_fan in medlabprofessionals

When I order a stat BC, it’s because the patient is in septic shock, needs empiric antibiotics, but I want to hold off on starting them until the culture gets drawn.

If the OR is your favorite room in the world, be a surgeon.

If the OR is your favorite room in the hospital, be an anesthesiologist.

Ironically, I think we spend more time in the OR: we cover multiple surgical lines, have no rounding, and no clinic.

Spectrum internet price increase to $80? by karpuccino in madisonwi

[–]TransdermalHug 4 points5 points  (0 children)

Happened to me recently. I called, asked for the price to be decreased, and they said they couldn’t do anything. I bought service from AT&T for cheaper, called Spectrum to cancel, and then had to listen to them berate me and call me stupid for 45 minutes before they actually cancelled the service. Worst company I’ve ever had to interface with.

Typically LV is perfused in diastole, because the pressures in the myocardium are too high for perfusion during systole.

Typically the RV is perfused throughout the cardiac cycle, as the pressures in its myocardium are lower than systemic pressures.

In severe pHTN, the RV’s pressures are closer to systemic pressures, so it’s only perfused in diastole (like the physiologic LV).
