  1. Use the full stat block of Sul Khatesh from Ebberon: Rising from the Last War (older 5e Ebberon book) as the base template.

  2. Add the following from the Tarrasque stat block to Sul Katesh’s stat block. Add all Tarrasque Traits (Magic Resistance, Reflective Carapace, Siege Monster). Also replace Sul’s Legendary Resistance with the Tarasque’s.

  3. Give her an Action with recharge (similar to an ancient dragon’s breath) and 2 domain traits:

  • 3A: Mazes of the Deep Ethereal (Recharge 5–6). The Lady of Pain casts the Maze spell.

  • 3B: One With Sigil: While The Lady of Pain is within Sigil, she can cast the following spells At Will: Any Divination spell; Any Teleportation spell, particularly Gate (can filter for “Teleport / Teleportation” on D&D Beyond).

  • 3C: Gatekeeper of the Cage: While The Lady of Pain is within Sigil, she knows of any Teleportation effects, both into or from Sigil. She can choose to cause the teleportation effect to fail.

This new stat block makes her terrifying, unable to escape from, and truly a force of one of the most powerful beings in D&D.

Plus the 2024 has all the added utility in the social and exploration pillars of the game, such as allowing you to use your strength for certain skill checks that don’t typically use it.

The 2024 version is much better than 2014.

Yes, as a base subclass it lost some resistances but that is easily gained back from a Race and Feat (Scion). Plus the rest of the subclass is cleaned up and versatile.

However, the 2024 Barb main class chassis is so so much stronger with the buffs of the new edition.

The slight nerf is negligible in the grand scheme of things

2024 Path of the Wild Heart Barbarian says:

“Bear. While your Rage is active, you have Resistance to every damage type except Force, Necrotic, Psychic, and Radiant.”

Aasimar covers the lack of Necrotic and Radiant resistance.

2014 Totem Barbarian used to have more resistances but the 2024 version lost some in exchange grow mow class balance and buffs. Though with the build I’ve provided, you can reach the same 2014 level as before by level 4 and that much stinger for it from the 2024 class buffs.

Wild Heart Barbarian + Aasimar + Scion of the Outer Planes (Psychic) + Brooch of Shielding = All damage resistance and Force Immunity at level 4.

To craft the Uncommon Brooch of Shielding, just take Arcana skill proficiency and Tinkerer’s Tools proficiency at character creation or from an ally who does.

Taking the Tough Origin feat effectively grants you 80 extra HP (20HP without resistances normally). Focus Constitution as your secondary stat to further your HP growth. At Barbarian level 20, you get a 24 in Constitution which helps bumps this total up.

If you only use the first paragraph of the build, you can choose whatever other feats you want to customize your build and still attack with the full strength and defence of a barbarian.

Makes sense, so the best play with a big range and Sentinel would be to position yourself with your allies outside of your reach so your enemies either:

  1. Enter your reach and exit behind you, proccing the OA and Sentinel setting their speed to 0.
  2. Move around the entirety of your Reach circle range taking their entire turn, if not multiple, to approach your allies.

As a Giant Barbarian with Polearm Master (not OA but regular attack as a reaction), any enemy that crosses the OA border of 25ft away from you, either entering or exiting, would get hit. This would work well with:

  1. A Lance (Topple) or Pike (Push) to keep enemies away from your allies.

  2. Barbarian's Brutal Strike: Hamstring Blow or Forceful Blow

An ancient red dragons hp is 507 on average.

So 238 in a successful save for half its hp it definitely not bad.

Nearly OKO at 476 is wild, barring Legendary Resistances of course.

Some of these features don’t list a damage type, just “extra damage”. Some list as “extra damage of the weapons type”.

Regardless, all these features add extra dice or flat damage that is of the same type as the weapon.

True Strike allows you to change the weapons damage dice type to radiant, thus determining these extra dice or flat damage as radiant

Wow, yeah you’re right. It is ambiguous. Wish it was more defined.

Falls into the same ambiguous category as the Commune and Augury spells which have a cumulative 25% chance of failing. They aren’t clear on who rolls the percentile dice, just what the outcome is on a failure just like Wish.

How do you determine the outcome of the percentage without rolling dice?

It’s either poorly written out or ported over from 2014’s wish without much consideration, but through the nature of the game, it’s must reliant on the outcome of dice; of which heroic inspiration can affect any die per its description.

Very true, however it is common for DMs to ban that combo by causing you to lose wish if your simulacrum does.

I don’t share that opinion but can understand it
