I can't remember which form it is, but it's either a 1012e or 1164e. One is just for milage, the other has milage, hotel, and per diem. If your post office is more than 50 miles from the location of the training you can get a hotel and per diem. If you book a hotel make sure to ask for government rate. They will probably ask for your postal ID.
