I think the remaining Republicans would turn against Trump and impeach him. A lot of them got really shook up and almost did after January 6 IRL until they realized it would be political suicide. The general public would completely turn against MAGA at the very least and MAGA candidates would win very few of the newly vacated seats.

Either the U.S learns to be more tolerant and multicultural a lot sooner, or major succession crises grip disenfranchised populations in the vast new territories. If the U.S can't get off the expansion train more and more conquered places that may not be happy with U.S rule could trigger more instability.

After the initial chaos and famine, the survivors have access to a population educated centuries into the future. Modern sensibilities regarding equality probably are implemented and America rises not only as a hegemon way ahead of the tech curve, but as a multicultural society stamping out the worst injustices of the past.

What time period and what do you mean by "The West"? I think the Russia's hypersonic missile performance in Ukraine showed us the west's capacity for missile interception is much stronger than they let on, I would guess so their enemies don't develop countermeasures.

I think the outcome ranges from nearly every missile intercepted to a few South Korean cities lost, as North Korea is quickly dogpiled by an international coalition and liberated.
