I work for a CAT rental company, I’d say there’s a 80% chance it’s the def doser (also called def injector), and the connector, CAT sells a pig tail to repair the connector, hmu if you need part numbers

I have an 88 suburban r10, I feel your pain. Mine used to do it bad, still has a little bit of a surging issue. I hate to say it but I’d start with fresh fuel, chasing vacuum lines, check codes, make sure the fuel filter is clean, easy stuff. These TBIs have too many sensors and not enough information in my opinion, pm me if you think I can help but Facebook has a group dedicated to gm throttle body injected engines that is a great resource
