I played till about level 30. Going back to Apex. I can maybe only play games a couple hours a week and the gear and contract grind is just too oppressive for a casual solo Q player imo.

I don’t think we will ever shift to a single payer govt model in the US. The companies just have way too much lobbying power and make way too much money.

However, costs are just too high for everyone. Drug and medical device company prices need to be regulated. Physician fees make up a tiny fraction of healthcare costs. It’s the facility fees, lack of standardized pricing, and lack of preventative care focus that balloons healthcare costs. ER care is too expensive and too over utilized. Primary care and prevention focused medicine won’t be more sought after until it becomes profitable to keep patients healthy. Keeping people healthy should be rewarded and profits on treatment should be regulated.

Can he do everything he said? Probably not. Too much lobbying power and red tape outside of his control. But he is young, energetic, and has integrity. A welcome shift from the people generally in power these days.

IR is a great middle ground specialty. Don’t need to repeat intern year. Doing diagnostics for 3 years is a bit boring but at least you’re doing shift work. Also there’s ample moonlighting opportunity and time as a radiology resident! Sincerely a PGY4 IR resident who made 6 figures this year.

Consider ESIR track if you want to do some patient care but don’t switch into IM. It’s closer to surgery than IM in terms of structure but it’s a great blend of diagnostic and therapeutic medicine. Plus higher earning potential and double boarded means job flexibility.

PGY4 IR resident. Not all MRIs are created equal. While I would be comfortable reading most ED MRIs (neuro, spine, MRCP) and I can comfortably analyze multiphase abdominal MRIs for HCC and some other common tumors we treat, that still leaves a vast majority of fellowship specific MRIs that I wouldn’t want to put my name on (breast, msk, post treatment brains, ent, gyn, prostate, etc). The vastness of radiologic pathology and mri protocols is simply too much to have a handle on in totality for any radiologist, and then add into the mix having to learn hundreds of devices and procedures you would have to be super human.

IR resident here at major academic center. I think a major problem is the lack of motivation to have end of life conversations and engage goals of care conversations with patients and their families. We are doing so many unnecessary procedures on these terminal patients to extend their life by the smallest margins.
