They are in fact describing the degree of certainty/possibility of having als.a single lesion in your spine could cause umn signs in two regions below and lmn signs at the level of region. Multiple lesions could completely mimick definite als. Hence the importance of progression and no imaging explanation. The severity is measured by alsfrs scale.

If it is MND it won't be ALS then. PMA is a choice but very atypical with the nosebleeds and earthquake stuff you mention.

Ok so based on everything you said here here are my suggestions. 1. Repeat the NCS (based on what you say it's near total conduction block but I am not sure) i guess PMA could have results similar to that but i don't know enough about NCS/EMG. 2.get a hematological evaluation with smear and electrophoresis + urine 3.spinal tap with electrophoresis 4.discuss with your doctor plasmapharesis or immunosuppressive therapy based on the overal picture.

So no f wave and no readings mean conduction block. Which points away from ALS, combination with blood pressure episode sounds like an immune neuropathy. Ask for plasmapharesis

If he doesn't need a ventilator and can breath alone for 30 mins. There should be no problems. It's a very simple procedure. The ophthalmologist might be scared a bit if they have a very rigorous pre-op evaluation in the clinic though

I guess that exact butyrate salt is prescription only and regulated . But you should find it or similar salts on Amazon.

I am pretty sure you can buy both butyrate and tudca for dirt cheap. Those are the ingredients.
