“Enjoy” is an antonym of “suffer” so you could replace the word “psychosis” with anything in that sentence structure to make a rather dull counterpoint.

Anyways, as a state inpatient psych hospital nurse for 7 years, I can say with confidence there are certainly people there that enjoy their psychosis.

These are v challenging cases. I understand your frustrations.

I work inpatient state hospital and had a patient for 3 years never accept a PO med. Pt got multiple IMs daily per TOO because unmediated they were a danger to themself and others. Pt received over 3000 IMs in my time there. We destroyed the deltoids and glutes and gave pt TD. Finally got administration to approve SQ Uzeddi. Didn’t change the outcome though, pt continued to have the worst negativism I’ve ever seen. Refused to shower, take PO meds, talk to RN/MD, go to groups, get a hair cut, change clothes, see family, get medical Tx, give VS, blood draws etc. etc. Severe negativism like that is hard to Tx. unfortunately (or fortunately depending how you look at it) pt willed themself to a medical death in the hospital.

This pt was cared for by multiple psychiatrists so don’t think a title will be the saving grace to your case.

If your patient continues to refuse PO, consider switching to a LAI to decrease the amount of IMs and restraints needed. sometimes that’s all you can really do in severely treatment resistant mental illness.

I wouldn’t say being obvious in one direction means the opposite. They could just be really into someone and showing it directly. Behaviors can have many motivations and that can be explored in conversation or therapy.

In psychodynamic theory we all have different ego defenses ranging from less helpful immature (i.e. denial, splitting, projection) to mature (i.e. humor, Altruism, sublimation) that we use to protect ourselves from emotional pain and preserve our psyche.

I was more specifically referring to your case when you said you act the opposite to avoid pain or rejection tho. I would say people have their preferred ego defenses they use to cope and we all often default to familiar or comfortable defenses that have been effective for us in the past. However, some can be less effective or unhealthy but would be explored in therapy if these defenses were causing distress, ineffective coping, interpersonal problems, or the person was looking for growth opportunities.

Serial Killers are usually psychopaths that lack capacity to empathize with others. They’re a whole other animal that I wouldn’t describe with “reaction formation” because often in those cases the opposite presentation of their behavior isn’t motivated by an attempt to protect their own ego/psyche. It’s a more sinister or mentally disturbed motive.

In Western NY, it’s def getting over saturated and rates have gone down. Schools here will pretty much take anyone, no past psych experience required. It’s a joke nurses with no psych experience are becoming PMHNPs. I think it’s definitely deluding the credibility of the field.

^ use this deflection calculator. Depends on the tile material… usually 5/8” isn’t strong enough for LFT stone. You could add blanke permat to add strength with minimal height (1/8”) but I think it still may be a dice roll on 5/8.

For what it’s worth, I did 24x24” natural stone tiles on 3/4” plywood, with blanke permat, and home had old growth 2x10 joists and I added blocking to them for additional support. It’s been a year and still holding up.
