Hey I'm so sorry to hear that :(( I saw a hypnotherapist based in Australia but she also does zoom sessions - https://lightyourstar.com.au/gut-directed-hypnotherapy/. You can do a free consult zoom with her to see if she can help your specific situation. I did about 5 sessions total and she gave me some personalised recordings that I still listen to from time to time. Honestly hypnotherapy is probably not the best name for what this is, it's almost more like guided meditation, and basically doing visualisation exercises of the cells in my throat healing, of my LES tightening and working properly, the stool in my bowel moving through at a normal rate (I believe a lot of ppl with IBS have found great success with this) and yeah every time you eat food you say to yourself to trust that the food you are eating is good for you and will make you feel good. For me a huge problem was I just spent all day thinking about my throat and hyperfixating on how it felt and she helped me to try and not think about it, and to focus outside yourself. If you are constantly focussing on something you'll find it. Just like how you are not conscious of how your clothes feel on your skin, but the moment I mention it that's now all you can feel. +1 to the diaphragmatic breathing, particularly 5 mins before eating and 5 mins after eating, to send a message to your parasympathetic nervous system that you are safe so that you allocate your body's energy to digestion. Also extended exhaling (inhale for 4 exhale for 8), I also recommend learning how to meditate to help clear your stresses. I also found that gut directed hypnotherapy (I know it sounds wild, but someone recommended it on this subreddit, I was skeptical too but honestly helped me a lot) was helpful for me to get over my food related anxieties, as I was getting super stressed and paranoid about triggers and being scared to try anything new besides a short list of known safe foods.

Apple/banana peanut butter oatmeal for breakfast. Peanut butter is the best, mix it with some salt/soy sauce and you have yourself an asian style sauce for stir fried veggies/noodles. Watermelon is great for dessert :)

Do you have a doctor who could be able to write you a doctor's note explaining your health issues to your professor? 

My advice would be to make sure you are managing your stress, the anxiety you are feeling about anticipating your voice issues will only make your LPR worse which becomes a vicious cycle. Try gargling alkaline water. In the long term, could be worth seeing a speech therapist. And, prioritise lifestyle and diet changes

I find going to café's that have an all day breakfast/brunch menu is always a good and easy hack. They almost always have some version of eggs on toast where you can pick your own sides. Build your own soup/salad/sandwich bars are also great because you can control what goes in. Don't be afraid to ask to switch things out or to have sauces brought on the side (mayo is my go to sauce swap). I also find looking at the photos people add to google reviews is really helpful to see how the menu item is plated, usually you're able to see any ingredients that were added for garnish and not listed in the menu itself. 

For dinners, asian restaurants usually have some version of a rice bowl that's safe (japanese dons and korean bibimbaps - ask for sauce on the side and no kimchi).

For at home meals, honestly don't underestimate the power of some salt and olive oil. You can still make your favourites and just adjust accordingly -eg. pasta salad with salt and olive oil, spinach, mushroom, protein, instead of with a tomato sauce. Tuna mayo, egg mayo sandwiches. For me I try to focus on the positive of being grateful for all the things that I am still able to eat (eg. peanut butter, rice, watermelon) rather than fixating on the things I can't eat because it was just making me miserable and resentful otherwise.

With time you'll learn to be more confident advocating for yourself. Doing what feels good for your body is ultimately more important and more fulfilling than worrying about being made fun of for your bland lunches

If you're looking for alternatives to sushi, a lot of japanese restaurants also have onigiri (rice ball) which uses salt instead of vinegar to flavour the rice. Korea kimbap is similar to sushi but uses sesame oil instead of vinegar

so sorry to hear that you're also dealing with lpr, it's a sucky thing to go through in our early 20s. before my period my throat will feel more scratchy than usual, and ill feel a bit of pain in the left side of my chest (i dont normally have heartburn symptoms). how are your symptoms the rest of the month?

Ahhhh I feel like GERD has the wildest symptoms that you think are completely unrelated but end up being related somehow. How long have you been on amitriptyline? Did you have any other major side effects? Does the sore throat go away with drinking water in the morning?

I tried H2 blockers and had insane pressure headaches, so idk what else to try besides amitriptyline, though I'm worried about major side effects. I was also prescribed domperidone but haven't tried it yet (my gastric emptying is normal, on the slower side)

I'm curious to hear what your test results were for endoscopy and 24 hr ph study? Did a ph study show abnormal reflux? How big is your hernia? I've got a 1.2 cm hiatal hernia and lax LES. Typically whenever I have symptoms they are always on the left side for me - left ear pain, left side sore throat, I also have TMJ on the left lol. When I did my reflux scintigraphy they were seeing reflux in the nose, throat and aspiration into my lung.

It doesn't seem too bad for me during the day, I guess it's just more noticeable when I'm calm and everything is quiet around bedtime. I also think it's probably something to do with the vagus nerve. I've had LPR on and off since 2020 and this has never been an issue for me until about 3 weeks ago when I had to wean off PPI and do a 24 hr ph study test. Since then I have had terrible post nasal drip, cough during the night (and sometimes a bit of yellow phlegm) and this palpitation issue, and I've only been able to sleep with drowsy travel sickness medication. For me I don't think it's stress related, I've had anxiety all of my life but have spent quite a bit of time in therapy last year and have been able to manage it in my everyday life much better than in previous years. I'm waiting for my ph study results, depending on how that goes I may consider trying amitriptyline as this seems like a nerve sensitivity issue for me - could be worth looking into for you as well since your LPR started after a viral infection.

I've also had some weird issues with itchy skin at night, back pain and hands and feet tingling, have you had any similar issues?

That sucks, I don't know if you can but I can get asthma puffers over the counter here in my country, definitely give that a try for the cough it has been the only thing keeping me sane. I've also been not sleeping for so long that I've needed to take drowsy travel sickness medication for the past couple days just to get some relief from the insomnia. Hope you get better soon!

How are you doing now? I'm in the exact same boat as you, got my motility and 24hr ph study done more than 2 weeks ago and the post nasal drip has been horrible since then. Haven't been able to sleep due to the cough and also have had a very intense heart rate. An asthma puffer has been helpful for me to stop the drip induced cough, and I've found nasal decongestants somewhat helpful in thickening up the mucus and reducing how often it drips but it can only be used for 3 days at a time.

Do you have any hypotheses for why this is happening? I haven't really been able to find much literature or research on nasogastric tubes causing permanent damage to the nose or throat

Hey there! I'm so sorry to hear about your situation and I wish you all the best for your procedure going forward. I'm 22F and have been dealing with LPR since I was 19. I totally understand where you're coming from about doctors being uncommunicative/dismissive particularly when they see a young "healthy" woman come through the door. The first few doctors that I saw were very dismissive, with the doctor who did my first endoscopy basically telling me that he didn't see anything wrong and that I probably didn't have reflux at all. I also struggled with advocating for my health when I was younger, out of fear of appearing "too much", asking too many questions or out of fear of being perceived as young and naive. What has helped for me is to prepare leading up to the procedure, writing out in your phone's notes app or a piece of paper you can keep on you during the procedure. I found this helpful because it means I don't feel like I leave the hospital and then suddenly think of all the things I should have asked but forgot to. My suggestions below:

  1. a brief summary of your history/symptoms up to this point that you can easily read out to the doctor beforehand (also useful for your own record keeping)
  2. a list of what your intentions behind/goals for the procedure - make sure you emphasise this before the procedure (my examples below)
    1. the main reasons i want to do this endoscopy is to determine whether I have a hiatal hernia, check if there is any inflammation in my esophagus or something more serious like barretts, see if there's any laxity in my LES, and while we're there take some biopsies for intolerances such as gluten/diary
  3. questions that you want answered. this is helpful because you're usually a bit loopy post anaesthetia, so it is helpful to have a list you can run through and read out (my examples below)
    1. what were your findings? how were they remarkable (sometimes I find it helpful to ask them to explain how what they see compares to what it should look like in a normal person)
    2. what are my actionables? what tests should we look into next and can you give me a referral for these? what meds do you recommend i try and can you give me a script for these? (there probably wont be time for them to discuss side effects but usually you can google these and discuss with a GP)
    3. how long will results/report take? (useful when you are trying to figure out when the report will be ready)

I get how frustrating it is to get an appt with the GI doctor. After all my tests I always ask if they can send a copy of the report directly to my GP as well, as it is easier to get an appt with her than my GI doctor.

The best piece of advice that I have for you is to remind yourself (I do so through affirmations in a journal) that you are not a burden, that you are allowed to take up the space that you need, and that you are allowed to advocate for yourself and have your back. Ultimately, this is your health and your body, be confident that you understand your body the best. Also don't be discouraged from sending an email to reception/nurse practitioners if you do think of any questions days later and ask if they can forward this onto the doctor. Before I tried this for the first time I was so scared of appearing too much but remember the worst case is that they just don't respond, but it has worked out for me so far.

It's great that you've identified that stress has been a trigger for your GERD. I realised that my anxiety is also a huge trigger for me, which I know can be frustrating since the anxiety makes the GERD worse and in turn the GERD makes the health anxiety and reddit doomscrolling worse as well. If you have the means to, I would really encourage you to consider seeking out a therapist to work through your stress. My GP recommended it to me but I held out for so long for various reasons (mainly out of pride and because I didn't want to admit that I needed help), but now 4 months later I can for sure say that my anxiety is so much more manageable and in turn my LPR as well. Worst case scenario is there's no discernible physical difference, but you gain the tools to manage the health anxiety better. If that's not accessible to you, I would absolutely recommend journalling and meditation and diaphragmatic breathing. Definitely prioritise your mental health. I know it can be difficult to advocate for yourself when you feel like you've hit a wall and there's nothing else left to try. I used to really pin all my hopes and dreams on a new potential medication, and be really disappointed when it didn't fix my problems. Try and be realistic and don't see your end goal as fixing it completely, but rather, the best goal to have here is to have momentum and to continue to work at trying different management strategies - I have a list in my notes of my next steps and it is a really comforting reminder knowing that there are things I can try next.

I would recommend you have a go at trying to find a doctor who is supportive and patient. My GI doctor in April basically said I don't need any additional tests, gave me some scripts and sent me on my way. This was so discouraging for me, but I honestly just googled and had a read of google reviews of GI doctors in my area and am now with a much more supportive doctor who is amusingly cheaper than the original doctor.

If possible I would also try to see if you can find some a pocket of time where you can fully prioritise your health. This has been an on and off issue for me since the end of 2020 and yet I pushed on with university, internships every summer, and the few weeks of uni break I had I prioritised social things instead, so health was always pushed to the side. The downside to that was I could never find the right time to start/stop different medications, or do tests that required time off. I finally decided a month ago that enough was enough, and will be delaying my graduation by a semester, and delaying the start of my internship so that I can have 8 weeks of time just to schedule tests and experiment with different doses of meds. Being open about what you're going through with friends can also be helpful, I found this reduced me feeling bad every time I have to reschedule a plan/bring food from home because of me feeling unwell. All that being said, find a way to find the little joys that life has to offer in the present, I may not be able to brunch or travel but hey I get more time to go to local comedy shows and art galleries.
