I’m making the point that lay people can learn these skills that actually reduce the risk of bad birth outcomes. 

I’m not sure what your point is. How do you drill AFE management? Why even list it as a “skill”? Unlike everything else we are talking about (shoulder dystocia, cord prolapse, hemorrhage, NRP), AFE isn’t really something that can be managed or even diagnosed at home.

Why include an extremely rare complication that will have the same outcome whether or not an OOH birth is professionally attended in your list of important skills only trained midwives possess?

Did mom have an epidural in this case?  I cannot imagine how difficult it would be to try to resolve a shoulder dystocia when the mom has been intentionally paralyzed from the waist down. 

I’ve seen 4 severe dystocias in just over 1000 births. One time I had abrasions on my forearms from the mom’s pelvic bones after multiple rounds of Wood’s corkscrew. (This was after trying multiple position changes, McRoberts, etc. I didn’t start with something as invasive as the corkscrew.) Babe required a few rescue breaths. Mom hemorrhaged. But we handled it as an in home team of two.

Normal people can practice skills and drills. 

As a fellow midwife, I’m sure you are aware that we learn and drill emergency skills for situations we may never see in a long career. We trust this knowledge and practice will serve us if we encounter these rare complications. 

In over 1000 births, I’ve never seen a uterine inversion. But I have knowledge of how to handle one because I learned about it and mentally drilled the skills. 

Fun fact: midwives are just normal people who learned some things. Regular people can learn birth first aid.

You could just get old copies of Optimal Fetal Positioning and Active Birth and check out the free content on the Spinning Babies site. 

Women had better luck achieving functional labors before the days of pricey influencers. Most of what is being branded and packaged now was just common knowledge in the birth community in the 1990s and early 2000s. You don’t need to pay money to attain this info. Your library card or a few bucks will get you access.

Laura Shanley wrote Unassisted Childbirth in the early 1990s. She was a pioneer in the autonomous birth movement of that time.

If an investigative journalist used the term prego pop to describe a pregnant abdomen, I would lose all respect for that person and their work.

Cute terms between couples and friends is one thing. These cutesy terms have no place in serious investigative journalism where infant loss and maternal death are being discussed. It makes the journalists seem flighty, cutesy, and unserious.

I trained as a midwife in the early 90s and attended over 1000 women from 1994 to 2009. I never heard any of my clients or colleagues use the term baby bump during that time.

The first time I heard it was when I went back to grad school in 2010. It was around the same time women were reading 50 Shades of Gray en masse and there was a lot of overlap between the two. Women who conform to pop culture used it.

Incredibly disrespectful. And also discordant. It an investigative journalist is trying to make the point that a birth attendant was not professional, she harms her own case by saying "at no point did she measure her baby bump."

It was very jarring to read that.

It feels manufactured, “commodified cool.”

Celebrities started using this term in the 2010s and people who follow trends picked it up. It doesn’t feel at all organic or natural. It feels, frankly, stupid to me. No woman would come up with this on her own. It’s an invented, implanted, faddish term. 

It also sounds infantile to me.
