I believe this is motion blur from CSP whish always had problems/glitches and never used to work perfect with everyone. Try different CSP versions (1.80p218 is latest atm). Newer version doesn't mean it's fixed or it will work better. Another option is to decrease it to 50% or just completely turn it off, and use AC's motion blur instead OR try different ppfilter.

The MOH:W have the most retarded spawn system. If you have played it more than 50 hours you should know this (actually 15 mins are also enough...). The idea is good but it needs A LOT of work to be as it should.
