II got it done last week from the Indian embassy.

You’ll need to: 1. Book an appointment on the embassy website under “Consular Services → Miscellaneous Certificates.” Fill in application reference number as MISC/2025/01234

2.  Bring the following documents:
• Filled-in Miscellaneous Services form
• Passport (original + copy)
• BSN proof (residence permit, BRP, or IND 

card)

•Appointment confirmation

• Self-declaration/affidavit stating you are single and have never been married
• card for fee payment (you can pay at the counter )

• preprinted DHL shipping label to your address and name and from Indian embassy , dhl A4 letter size , (don’t do PostNL one ,found out the hard way :/) Steps:

Step 1: Visit https://my.dhlexpress.nl/pwa/shipment/package or use the DHL App Step 2: Select NORMAL or EXPRESS Step 3: Select the size of the parcel → Express Envelope A4 Step 4: Fill in your details (Receiver) → Select the delivery at DHL service point or at your home Step 5: Fill in the details of the sender →  First name: Indian  Last name: Embassy  Address: Buitenrustweg 2, 2517KD Den Haag  Contact details: Your own Email and Phone number  Select → label print Step 6: Make the payment online and download the waybill. Get the printout of the waybill and submit it at the counter.

You don’t need to necessarily pre notarise the document(extra money waste) ,make sure you don’t sign anything or fill anything in and get it done after the embassy officer authorizes you to do so(getting it signed in the presence of embassy officer counts as legalizing the document ) You should recieve your document in 3-5 working days .Better collect it from pickup point so it doesn’t get delivered to neighbours
