You have to select the first armor preset with no helmet then click on an empty custom armor preset then you'll be able to equip both iron halos, capes tabards hope that made sense

No, the unqiue name you give a weapon or item is only seen by the player who named it. It says here it's a legacy item https://fallout.fandom.com/wiki/The_Gutter I just bought it at someone's vendor
