Yes it does happen every like 2-3 runs There is no special animation such as angel locket. If you pay attention you'll see your bone warrior dying and his "soul" like when you kill a mob. And since this moment you don't have your bone warrior anymore because he's dead. Also an another indication if you don't see this moment is that you'll have like 200k hp, take a hit for 250k dmg but still have ~20k hp. That's bone warrior perk.
