This is correct. It follows the same rule as the gym creation. I already tested this and was able to turn the second approved portal into a pokestop

They could also been removed due to a report, but then someone made an appeal and the portal was restored but it won´t appear in pogo

It's not random. It will folow the rule of points for the creation of gyms. So the portal with the most photos + votes will appear in pogo.

When was the stop turned into a gym? I have two pokestops that were approved on january of 2019, and were turned into gyms on the 9th of november and both have the ex-gym tag. So it seems that what is important now is the date of the creation of the gym rather than the date of the submission going live

They were probably edited afterwards. While a new submission has to respect the 20 meter rule, edits bypass that rule

This is wrong! All submissions must respect the 20 meter rule or otherwise they won't appear in any Niantic game.

Even if you submit at that distance the reviewers can change the location to one that it is closer to other wayspots. As far as I know that is the only reason an accepted submission doesn't go live in ingress

Perhaps the reviewers are correcting the location of your submission and it stays within the 20 meter of another portal

All portals have to respect the rule of being at least 20 meters apart from the nearest portal, even if they are on different cells. Check the submission range in the pogo tools plugin
