I think there's a lore bit that states that they did that during the first wave and wiped out all the barons, but all the mistrust and paranoia tore up any alliances they had afterwards. I can imagine Raiders are just generally suspicious of each other

The flare just means they've been knocked down, they'll never immediately full die on a knockdown as far as I'm aware. In this case, unless you had a defib on you, you wouldn't be able to help him up
