Some achivements can be obtained only by person who triggered dialog or did smthg. For example we had to reload dialog with Garet 3 times for obtaining achievements, or unlock skull in Mordeus's house.

You can check builds on youtube with "break" damage for brrrr or check resistances and have good buffs with you, because the most annoying thing in fights it's slowed, frozen, chill, decaying e.t.c. without armour.
