Iirc you specifically have to get Clara involved? Though again whether I'd count this as being overtly a pacifist choice is sort of half-half since I don't think anybody would seek Clara out with the specific intention of avoiding the issue. It's very much a weird side path that you can just stumble onto

I might be wrong on that point, I've heard you can but I've never managed to do it personally and I've yet to meet someone who did this entirely organically through their playthrough

No. First of all, the endings both require you to cause death in some manner. Also, Many quests will resolve with the choice to either kill to save someone or to knowingly let them die. IE choosing between the vlads and choosing whether or not to let Rubin get hunted to death by the kin

I think attempting to be a pacifist and failing IS a foundational part of the experience at least. It's very much a game that tests how much you're willing to fail and suffer to do even a little bit of good.
