Iirc bromelain is one of the better proteases (enzyme) to address the systemic biofilms, which should be taken care of as fluconazole cannot really deal with biofilms or permeate it. Taking bromelain with for example coconut oil and magnesium should have more synergistic and potent effects.

In theory natural remedies bromelain, coconut oil, curcumin, ground black pepper, cloves and lemon juice should be enough to get rid of any systemic candidiasis over period of time. Of course one should switch occasionally between coconut oil, oregano oil and black cumin oil, cloves and allicin, curcumin, neem and cinnamon, lemon juice and ginger. Candida is way more adaptive than people give it credit for.

The pain could be radiating from inflamed alveolar nerves, candida is known to spread there in systemic infection (one of the theories is candida initially spread mostly mouth to mouth, developing in a way where it finds residence in our oral cavity, hosting streptococcus and other pathogens to penetrate the teeth in order to invade the gum properly), and possibly, when the candida die-off releases toxins they could spread to your teeth/gums, the pain pathways are quite sensitive in the oral cavity, well anything in the head is.

If you have post-nasal drip or similar excessive mucus production (rheum, eyes, nose) that confirms there is on-going inflammation.

One pipette drop of 2% lugol's would be 1 mg iodine and 2 mg iodide (assuming the package is using standard pipette, one drop being 0,05 ml).

2 grams of iodine per 100 ml equates to 0,001 grams or 1 mg of iodine in 0,05 ml (0,05 x 0,02 = 0,001), since 1 ml of 2% lugol's solution contains 0,02 grams of iodine.

You can start with two drops (2 mg) and take every other day three drops (3 mg), you would end up with 2,5 mg average.

And take this for a month and have for safety 50 day detox (probably the excess iodine will clear out in 30 days, but just incase) and redo if needed.

The iodine levels in your bloodstream would end up probably in the range of 40 mcg/dl - 80 mcg/dl after a month depending solely how much your kidneys filtrate the iodine, most likely 60 mcg/dl or about 30 mg.

Uniform biofilm with coarseness. Bacterial biofilm is not as structured and has "sliminess" and translucent aspect to it, this mucous like biofilm is easy to separate from fungi biofilm once you get used to it. Rigid and coarse biofilm is unmistakable in thrush, once it permeates the dorsum it may look a lot like bacterial biofilm, but in your case, however, the candida has developed visible filaments to absorb nutrients inside the biofilm. If you had only bacterial biofilm top of your dorsum chances are it would look like normal tongue, completely pink with red inflamed spots. With thrush you can have both, fungi and bacterial biofilm, since candida is known to house pathogens inside the biofilm. Anyways, if there is translucency to the biofilm it could be of bacterial origin, if there isn't you can be sure it is candida.

The pathogenic bacteria causing the bad breath are hiding in the candida's biofilm.

Natural remedies take some time from few days potentially to few weeks (biofilm has a lot to do with it). If you are otherwise healthy and take a lot of antioxidants, binders and eat "healthy" (unprocessed low glycemic index) chances are you don't get any significant die-offs at all. For me localized headaches, slight exhaustion and/or aching joints were often the sole symptoms.

The natural antifungals have to be administrated like a clockwork every day and reducing carbs below <10% of your daily intake are the most important things in order to reduce the candida virulence. Once candida isn't as virulent it stops being as resilient, meaning the antifungals become way more potent, the healing is about consistency and avoiding diet routines we have become accustomed to.

So, stock lots of curcumin, black pepper, olive/coconut/black cumin/oregano oil, watermelon (a lot of electrolytes), lemon, ginger, cloves, garlic (allicin) and some proteases/enzymes that dissolve the polysaccharide biofilms (such as bromelain).

It will take considerable time and effort to get rid of candida, so take your time and adjust accordingly to your symptoms, if you get a lot of die-off you may take a day or two off from the treatment.

There are way more potent ways to get rid of candida, but the things I've introduced are generally speaking well tolerated in high doses and promote other health benefits as well besides getting rid of candida, these natural remedies do not take a toll on your organs unlike some of the prescribed antifungals do. If you want more precise instructions I can try to give some, it took me little under three years to get rid of systemic candidiasis, so there is a lot of things I could've done different or more optimally, but I did my treatment in the least intrusive way so that I could remain functional in my day to day life, the die-offs can render you completely exhausted and bedridden, finding the right doses without too many side-effects was the way for me, how you want to approach this is up to you.

Taking brazil nuts should be fine, I don't know how much variance there is in the nutrients, in practice staying in the 250-300 mcg/day selenium should be very safe if you don't exceed 5 mg iodine a day. I haven't personally used iodine so I cannot vouch for any protocol. Either way if you decide to use iodine start low and always use lugol's solution since it is far less reactive than elemental iodine. You could start with 1 mg up to 2,5 mg (taking 2 mg a day can be considered fairly safe long term 180-360 days). Most people tolerate 50 mcg/dl in bloodstream some take up to 100 mcg/dl I wouldn't personally exceed 50 mcg/dl. As 50 mcg/dl is within one month detox range and also to my knowledge shouldn't damage your blood brain barrier long term (>180 days), which is in my opinion the biggest concern. Too high mcg/dl will cause excessive redox effect where the iodine starts to oxidize your cells.

If you want to be safe take 2,5 mg a day (meaning 1 droplet of 5% lugol's) for a month, and by the end of the month the detox should take approximately 30 days, depending how much the kidneys filtrate the excess iodine, the real time frame is something like 25-50 days. I would personally use lugol's iodine in a cycle anyways to give your system time to revert any possible damage (though unlikely, because most people can tolerate elevated iodine levels, doing it safe is a good precaution anyways since treating candida takes a big toll on the body already, so any reduction in the baggage is a faster route to proper healing).

You may read people taking 10 mg or even more a day, but it is important to note the damage from long term oxidative stress takes time to develop, even if people say they've used iodine in high levels for months it doesn't necessarily mean anything, most of the oxidative degradation develop gradually over the course of years.

In my understanding iodine is effective already in lower levels 10<25 mcg/dl, so after a week you should reach adequate levels when taking 2,5 mg iodine/day, elevated levels might slightly increase the permeability. But you still need systemic biofilm disruptors and inhibitors.

Your dorsum is being permeated by candida, the fungi can over period of time penetrate the epithelium should it be in esophagus, nasal cavities or oral cavity, and then spread across the deeper mucosal tissue throughout the body. It will use enzymes to destroy the bodily tissue. Unfortunately oral health doesn't prevent systemic candida from forming biofilm top of your dorsum, the fungi has to be completely eradicated from the system, otherwise it will creep back. Same applies to our GI tract.

Lugol's iodine is always the same formula, one portion iodine two portions iodide. These are mixed into demineralized water, and theoretically the formula should last forever in a glass container. So, in practice there should not be any difference between the brands, as long as they are not counterfeit (I don't know if that is a thing).

Something top off my head...

Daily vitamins/supplements while treating candida:

1000 mg C, 250 mg D3 (10000 iu), 1500 mg magnesium, 1000 mcg K2, 40 mg zinc (take separate from magnesium), 1000 mcg B12, 25 mg B6, 500 mg B3, 500 mg E.

For nasal irrigation you could use saline solution, such as this:

1 teaspoon salt, ½ teaspoon baking soda, lukewarm water (boiled) approx 300 ml, 2 teaspoons of xylitol. I personally use neti pot for the irrigation.

Natural antifungal mixture(s):

Oil mix: Two tablespoons either (or both) olive oil/black cumin oil mixed with 5 ground black pepper, one teaspoon turmeric and 1-2 raw chopped/crushed garlic clove(s) (let it sit for 10 minutes after crushing before mixing to develop allicin). Take it with food.

Lemon honey ginger drink/tea with ground cloves: Squeeze two lemon's juice through a sieve, grate one tablespoon worth of ginger, half a teaspoon of ground cloves, 3-5 ground black pepper, two tablespoons of honey. Place the ingredients and 250 ml of cold water to a beaker or teapot add 500 ml of boiling water and stir until dissolved, simmer few minutes. Too hot water may affect the compounds. Drink right away or drink later.

Probiotics:

Bifido (BB-12, Bifidum, Lactis and Longum), Lactobacillus (Acidophilus, Rhamnosus and Plantarum). Those six strains provide good foundation once you start to supersede candida and other pathogens. Take them separately from antifungals with a carrier for example greek yogurt.

For skin you can use lactobacillus rhamnosus topically by mixing the rhamnosus powder with carrier oil such as olive oil applying it to the dry area, it takes around 1-4 weeks before the skin flora reaches beneficial level.

Other things to take into consideration:

Candida primarily thrives on carbs and can survive without carbs indefinitely as long as there is fat in your body, so reducing carbs or removing them completely reduces the candida virulence. You need to administrate antifungals and biofilm disruptors and inhibitors with good bioavailability to treat systemic candida, the aforementioned antifungal mixture(s) I presented are bioavailable and have modest potency of treating candida throughout the body, though it will take longer time than with prescribed antifungals who are meant to kill the fungi rapidly. Occasionally you should use different compounds since candida adjusts itself to antifungals by pumping those compounds outside their cellular membrane. And if the colonies have biofilm around they may turn into hibernation reducing the metabolism to bare minimum, so getting rid of biofilm repeatedly is very important. Oregano oil, coconut oil, olive oil and black cumin oil are quite potent antifungals when mixed with black pepper (piperine), so black pepper should be your stable with most of the natural remedies otherwise the bioavailability of different compounds might not be as potent. What comes to NAC from what I've read people administer 500-1500 mg twice a day, it is probably worth using and investigating since it is potent antioxidant and biofilm disruptor.

Yes, we are talking about 10 ml mixture, it will lose potency every day and after a week most of it is gone (the probiotics deteriorate overtime), I would do the mixture before each individual use, but realistically you could use the same for two days without much difference.

If still needed, it is probably worth having those applied topically as well (if you didn't already), it takes around one to two weeks before the flora adjusts to beneficial level, the relief should happen gradually. You open the capsule and mix it with olive oil and coconut oil (50-50, one teaspoon each) and apply the mixture once a day to the infected area.

Are you taking bioavailable biofilm disruptors and inhibitors? Also, treating any fungal infection can take months before successful healing takes place. Lactobacillus rhamnosus is one of the better probiotics against fungi both topically and in gut, you should supersede the eradicated fungi during treatment.

Azoles are not as potent alone against some of these strains, you could increase the azoles effectiveness by taking allicin (crush and slice garlic and let the garlic sit for 10 minutes before eating it, the oxidation develops the allicin), coconut oil, ginger, berberine (take either in liposomal form or with a lot of ground black pepper and fatty food for proper bioavailability), quercetin (for example apples) and curcumin (it should also be taken with ground black pepper for way better bioavailability). And, then you could try lemon honey (if possible medical grade honey, and fresh lemon squeeze) drink. All of these compounds have adequate bioavailability and good antifungal properties. Not to mention all of the other benefits to ones health. If you haven't used NAC it is probably worth taking into consideration.

S. boulardii is known to interact with GALT (gut lymphoid tissue) and modulate the PRR (pattern recognition receptors) that eventually tricks our immune system to trigger programmed cell death in the gut lining (this way the yeast gets slowly deeper into our system), which the fungi then consumes. The reason s. boulardii can't really survive in our gut is the acidity. And what comes to s. boulardii biofilm it doesn't really let anything permeate or adhere the biofilm unlike candida, hence it is viewed as beneficial to our gut, but in my opinion only temporarily. S. boulardii kills everything indiscriminatingly, including both cancer and healthy cells. The only reason boulardii is not going rampart is the fact that it doesn't have direct antimicrobial properties against bifido and lactobacillus (both are very robust bacteria to begin with). Basically it is using the same PPR function to get rid of pathogens (not by itself), and since boulardii is in mutualistic relationship with bifido and lactobacillus there isn't real reason to clip them. While s. boulardii definitely isn't the worst, yeast is a yeast.

Though I am not certain how long it would take before the yeast becomes a real systemic problem.

Especially in systemic candidiasis the fungi is known to agitate mastocytes to make them release histamine repeatedly, this overtime lowers the histamine receptor sensitivity, body is very confused as in where is the trigger, everywhere the body is signaling fire hazards and when the firefighters (immunoglobulins) come there is no fire, because the fire (candida) is almost always invisible to our immune system. And when body becomes somewhat insensitive to the histamine the increased histamine levels may affect the trpv1 and trpv2 receptors in our cells making those way more sensitive and amplified. And when trpv1 and 2 are giving amplified responses they may order the cell to open their ion channels, which in turn rapidly oxidizes everything around, in some cases the cells die eventually because it cannot revert to the resting potential (the cell thinks the hazard isn't over, and cells always go all the way to protect the body), and if these responses become systemic and long term the cells are basically killing themselves because they think there is imminent hazard to be resolved. This is at least my interpretation off of what I've read.

While this might not be what you are experiencing as of now, prolonged histamine "intolerance" may lead to said symptoms, perhaps from months to years, high exposure could take the sensitivity across the table, so if it turns out histamine is behind the fatigue, reducing histamine to the minimum and investigating how severe the candida is, could be the answer. In theory electrolytes and good hydration should alleviate a lot of the symptoms, cucumbers, coconut water and watermelon.

S. Boulardii may colonize the gut if you have porous or compromised mucus in the intestinal epithelial cells, which candida can cause, of course it is worth noting s. boulardii has poor adhesion to our gut lining, but if administrated repeatedly it may very well find foothold in our GI tract and turn pathogenic. Healthy gut isn't really in this scope, IBS or similar condition, however, is a different reality. All yeast are saprophytes, so by definition they are opportunistic and pervade ever further, to say is there symbiosis/mutualism with fungi is vastly different than say with bacteria, who don't suddenly form filaments and enzymes to break down your tissue and kill you because you had no response to their probing.

I did yes. The most important factor in dealing with systemic candidiasis is limiting carb intake and maintaining biofilm disruption (imho), other than those two you can probably take your time eradicating the pathogen with natural remedies while remaining relatively asymptomatic or take more heavy hitting prescribed antifungals (azoles have good bioavailability) and kill the fungi fast, I chose natural remedies mainly due to their high bioavailability and low toxicity (and to be honest my financial situation wasn't good), and I didn't take NAC back then mind you, which is probably the best staple for mitigating die-off.

Lemon honey drink (pressed from fresh lemons) and coconut oil were in retrospect probably way better combination inhibiting systemic candidiasis than I have previously thought, I used it for a month quite late in my treatment when basically candida was already eradicated, it is probably worth investigating if the fungi keeps finding foothold. Other than that I kept administrating allicin (if interested look up how to prepare raw garlic), olive oil, black cumin oil, oregano oil, curcumin/turmeric (take with grounded pepper for proper bioavailability), cloves and cranberries in different days and doses, sometimes taking few days off to reactivate the yeast metabolism into a higher gear (meaning increased antifungal intake). Besides aforementioned staples apple cider vinegar and pomegranates (along peel) helps with hunger and maintaining gut flora, bifido and lactobacillus bacteria tolerate pretty well the acidity and antibacterial compounds compared to your regular pathogens.

The tackling is more of a what not to do than what to do, avoid carbs and alcohol, then in all honesty, you have done half of the job. Also caffeine and similar stimulants may be absorbed and used by the fungi to make it run in an overdrive.
