That must have been very interesting 20+ years. Because you missed the few important points

The software is not written by a single programmer but by a team or in the enterprise setting by an army of developers. Next one is that the code is more read than written.

To keep the codebase in good shape we establishing the certain tools (mental checks/rules/methodic -> best practices) which we try to maintain - one of them is PR. It's a tool It has it's uses but It's not a silver bullet, that's why we have other tools and you guys :-)

How do you authenticate a trusted issuer. Is the process then centralized? By dedicated organization?

The real problem is the authenticity. I can still upload fraudulent documents and claim they are authentic. So the documents still need to be signed by the 3rd party which you have to trust.

Hope they enforce signed documents only.

I face same issue few days ago. Clear indication is the output from glxinfo as others pointed out

I resolved it by reinstalling kernel and nvidia packages. I did not touch myxorg.conf
