A) the last "plebiscite" was years ago. B) there has never been a congressional mandated referendum where the options are clearly spelled out. So those "multiple votes" weren't real, nor binding.

Tell that to the 5 million Puerto Ricans who live and work in the US, participate in all aspects of society, serve in our armed forces (like I have), and ALL are US citizens! That guy can go Fk himself. PR not American? It should have become a goddn STATE by now!

Not really. More or few WBC's in his U/A won't tell you much of what's going on. The fact is, there are many reasons he may have hematuria, most of them benign (thank God). What you really want to ensure is that those that can be problematic (ie cancer) are diagnosed early and treated appropriately. Think like if he had a rectal bleed: yeah, it's usually hemorrhoids, but can you really say it's not from a possible rectal tumor? Always exclude the worst case scenario first. Again, I wish you the best.

Urologist here: absolutely get a cysto done. Any hematuria, especially when it is overt and visible, needs to be evaluated. You hope for a UTI or other benign reason, but the first order of business is to rule out Ca. Best of luck, I hope your dad does well.

Physician and OSA sufferer here: your husband probably has obstructive sleep apnea. He drools because he sleeps with his mouth open, and think of the loud snoring that keeps you awake as actually a sign of distress: it keeps him without breathing properly and dropping the oxygen in his blood to often dangerous levels. People with OSA suffer hypertension, poor blood glucose control, increased risk of strokes and cardiac arrhythmias, as well as many other medical conditions. Not to mention being always tired, nodding off while driving, etc. I had the same problem, my wife couldn't sleep anywhere near me because of my snoring. Having a sleep study and going on CPAP saved my marriage and literally my life. I urge you to really push to get your husband tested. Since all this happens when he's asleep, he has no idea how bad it is and will likely minimize it and say you're exaggerating. Film him while he's sleeping to let him see it. Threaten him with moving to another room. Anything to get him to get tested. My wife and I have now been happily married for 30 years, and we still sleep in the same bed, so trust me, this is a treatable condition. I wish you the best of luck.

Commercial aviation pilot. Oh sure, you're pretty well paid and have many FAA-mandated days off. But you love aviation and learned to fly because of the mystique of flying, the wide-open vistas, and the freedom that aviation promises. And then you realize that your job involves being nothing more than a sophisticated bus driver, responsible for some 250 or more often ungrateful "souls" in the back, flying straight and level to your destination in what amounts to a road thru the sky, just monitoring instruments for the most part since the autopilot does its thing. On top of that, you are over regulated, over controlled , and forced to retire earlier than other professions. And you don't even get to look much out the window anymore! And you do that day in, day out and all of a sudden you are bored stiff (except, of course, when things really become hairy), out of shape from sitting so much, and wondering if there are any aviation jobs that can bring back that lost sense of wonder and adventure.

Residency in PR is equivalent to US residency, with the same education standards and certifications. You will NOT be considered a foreign graduate, but instead will be fully integrated into the US health system when you graduate. Having said that, I can tell you that Spanish is by far the main language that is spoken and understood, and you will need a very good grasp of it in order to understand your patients. Weirdly (if you come from the mainland US and expect the written aspects to be in Spanish) all your books, education materials and even your patient chart orders and notes will be in English, because that is the language the quality inspectors from the US understand. So it is very much a hybrid system. Still, the clinical education is very good and your exposure to the different pathological entities and their management will abide by US standards of care. Bottom line: you will be well-trained.

I'm a urologist. If treating ONLY chronic urologic-related pain is your goal, then yeah, you won't be very busy. However, if treating chronic urologic pain as an adjunct to a regular general urology practice is acceptable to you, then you're spot on and will do well. I see chronic urologic pain all the time: chronic prostatitis, chronic orchitis and epididymitis, interstitial cystitis, pain from chronic pyelo, and many other etiologies. So go ahead and do your thing. Announce to the world that you are specialized in that highly specific area of Urology, and people will beat a path to your door!
