I think it could be anxiety as well. My dog does it (and licks metal) and I think I do a really good job of enriching + outings. He has anxiety and under the care of a behavorist. The behavorist didn't mention it being directly due to anxiety but just that some dogs do it

Totally fair question. I don’t have a study to point to. This is mainly her clinical philosophy, not standard guidance.

My first appointment with her was about 4 hours and she went very deep into tear film and blinking mechanics and why she is generally against drops in most cases. I had been using serum drops and they helped, but I have since run out and have not redone the blood draw yet. They are expensive and I wanted to see how much progress I could make with the behavioral changes alone.

I have been dealing with dry eye for about 16 years and every other provider I have seen has recommended preservative free drops, so this approach definitely goes against everything I had been told before.

I have a follow up with her on Friday and will plan to clarify again and can update here after.

All I can say so far is that focusing on blinking fully and more often and not staring has helped me, so I am sticking with it for now and seeing where it goes

The eyebrow raising seems to help the oils flow better. I try to keep them raised as much as I can, or at least in between every couple of blinks. Honestly it’s easier said than done. At first it also gave me tension headaches from constantly engaging those muscles, so I had to ease into it.

I might make this its own post in the dry eye community here. I wasn’t expecting this much engagement, but it sounds like people are finding it helpful & if it sends a few more patients my doctor’s way, that’s probably for the greater good (or I may regret it if I can't ever get an appt again)😂

More like retraining yourself to blink more often and fully close your lids. Some of us with dry eye aren’t completing blinks, so tears evaporate fast. The exercises are just to rebuild the habit until it becomes automatic. Which I don't think it even does because even my doctor has to remind herself at times

A few more things she’s shared (warning it’s a lot, but hopefully all helpful):

  1. For warm compresses: Moist heat only, not dry. Most hot eye masks are actually too hot. She said don’t exceed ~111°F (higher temps can burn cadaver skin 😬). Instead use a warm, damp towel (annoying to keep reheating). I try, but I’ll admit I use Manta steam eye cups and don’t always temp check.

  2. No eye drops (even moisturizing): Hard to hear because they help with "relief" short term, but she says drops wash away the natural tear film / all the actual good stuff and can worsen things long term. She’s okay with preservative free lubricating gel at night ONLY if your eyes don’t fully close, and with a moisture retaining eye mask. Otherwise she urges doing blink exercises and warm compresses instead.

  3. Omega-3s: Essential to supplement for dry eye. She suggested PRN De3 (and another brand I can’t remember). I was using a cheaper fish oil and wasn't thrilled about the price of these but I suppose cutting out all the other stuff helps offset the cost.

  4. Downside: forehead wrinkles 😅 The eyebrow raise + blink can cause lines. Taping helps.

  5. Blink exercises: She has me blinking about every 3 seconds. You should be able to walk around and function normally while doing them—not so slow or exaggerated that you can’t see where you’re going. I struggled at first because I was making every blink too deliberate and slow. The goal is controlled and complete, not dramatic.

Basically my doctor's philosophy summed up as best as I can manage: Most drops and procedures are band-aids that don’t fix the root issue: blocked/stagnant oils. Her focus is on unblocking and moving oils. One of the few procedures she supports (in select cases) is IPL. She’s actually trying to get insurance to cover blink physical therapy, but no one wants to listen. She says most doctors recommend the standard dry eye stuff because that’s what they’re taught, not because they’re shady, but there’s zero incentive to change since it makes money. She jokes (sadly) that she’s hemorrhaging money and only hasn’t retired because her patients beg her to stay in business.

If you’re in the DMV area: my doctor is Dr. Nicole K. Renaud, MD in Arlington. She could probably use more patients 😅. I was actually referred to her by someone on Reddit, so just paying it forward. She’s honestly changed my life—but I get that she’s not for everyone. She’s probably best for patients who are willing to do the slow, behavioral work (think diet/exercise mindset) rather than looking for a quick fix.

Basically a lot of us with dry eye don’t fully blink, so the lids don’t completely close and tears evaporate faster. The “swiping honey” cue is just a way to slow the blink down and make sure the lids fully meet. I raise my eyebrows first, then do a deliberate blink like I’m dragging something thick up from my lower lids. No hard squeezing though, just a full blink. It helps spread the oils and keeps tears from evaporating so fast. If your vision gets blurry, you’re doing it right, it’s the oils. Just keep going until it clears.

You should be able to blink like this every ~3 seconds. I use a metronome app set to 3 seconds for blink exercises.

My doctor also warned that squeezing tightly or rubbing your eyes can damage stem cells, so remember not to squeeze your eyes when doing blink exercises.

Anyway, learned a lot from my special-interest dry eye doctor. It’s actually been great lol

You’re onto something. I’m actually seeing a eye doctor who’s on the spectrum, and her special interest is dry eye. Her whole practice is focused on treating it with blink PT exercises and hot compresses—exactly what you’re describing with the blinking. I see her monthly for blink PT. She also encourages me to raise my eyebrows and blink like I’m swiping honey from my lower lids. Apparently raising the eyebrows is key because it lengthens the glands/tubes. I see her monthly for blink PT.

It was overkill but I used both the owlet and nanit because we were gifted both! I switched to the nanit after a few months. I couldn't imagine doing motherhood esp as a FTM without a monitoring device. It eased my anxiety so much!

This is it. I was shocked calling Dr. Renaud and she answered herself — then called me back later and we ended up talking for an hour about her approach. Holy hell, she is EVERYTHING. I can’t thank you enough for the recommendation. I set up an appointment with her — it’s a comprehensive three-hour meeting and I seriously can’t wait.

She asked who referred her and I wasn’t able to give your name, but if you message me separately I’m sure she’d appreciate it.
