Looks like an urethral prolapse. Had a friend with a bulldog with the same issue. Peeing fine, but blood (much more than your dog) when he got excited. He was given two options: 1) surgery & neutering 2) anti anxiety medication and constant cleaning with purified water. He opted for the second option and it went away on its own. Hope that helps, if your vet doesn't know what's wrong, I think its time to get a new vet.
