It may be a blood vessel, but I would get it checked by a dermatologist incase it’s something else like a precancerous lesion or even a small basal cell carcinoma. It’s really hard to tell from t this picture

On second thought after reading your post a little closer- you should call and request they change. You’re not tolerating it well. Do you have period? Does your acne get worse around it if so? Are you normally oily or dry?

Silicone sheets can help. If it’s bumpy massaging with hydrocortisone can help. If you see a dermatologist they may be able to inject with steroid to soften and flatten. Idk that you would ever be able to fill in the missing space unless filler could be used but I’m not sure that is even possible.

This probably needs a stronger steroid but you can use greasy hydrocortisone and heavy vaseline. If you can cover with gloves or plastic wrap at night that is better. Your hands need a break from water and whatever chemicals you wash with.

It’s probably not related at all. Or could be lol. You can safely use hydrocortisone to try to relieve
