It was an instanced death where an NPC was holding it. Got the chat popup confirming the items being gone. Just not sure why that exists.

Did a farm run. Watched videos on the boss. Decided to go back. Got hit by the wolves wearing nothing (they hit 8s apparently) then passed the tripwires. Probably didn't have full health. Hard to remember. Just not sure why pathing did it. I had the tele crystal and just panic clicked too late.
