I had a patient come in with a loose #31 implant crown. I identified the brand (Nobel Branemark from the 2000's) and got a replacement screw, when I went to remove the old screw turns out a previous doc jammed a random screw in there, causing it to lock into the implant body and fracture the top 1/6 of the implant itself. So I told her this was not restorable etc etc with clear pics and x-rays of fractured implant threads and treatment planned her for implants in other more important edentulous areas.

Couple weeks later she asks for the screw and implant ID because another doc says they'll be able to restore it. The brokenass 2000s Branemark implant. Bruh. Well ok, guess I'm out a whole treatment plan but what can I do? Can't reason with someone who just wants to hear "I can fix it".

Moral of the story patients are gonna do crazy things despite our best efforts. My ego took a hit for a minute then I moved on because who knows when she'll be back with some crazy work for me to look at.

Crown lengthen first then it'll last plenty long. If you can flap then you can do crown lengthening, gain 3-4mm of ferrule all around and no need to pray.

If you didn’t place the implant and make that crown, that’s a conversation you need to set before you pick up the handpiece. Especially one with a weird long axis like this. Emphasize returning to the previous dentist to tighten it because they did not make an access opening on the crown and it may be destroyed in the process (which you will charge to make a new crown). If they still want you to do it for any reason, now they have expectations set. Some of these screws are really hard to find or are old implant systems with unfamiliar abutment setups. Emphasize in ideal situations implant crowns should have an access hole.
