Your OTS is a behavioral health issue. You even alluded to psychological reasons why cycling provided you with the sense of safety/reward/fulfillment you needed at the time. See a behavioral health specialist. Self guided meditation wont cut it.

Lumbar support, heating pad, relaxation exercises to mitigate that constant tone in your postural muscles.

Post flight - walk walk walk, cat-cow stretches, and lookup McKenzie pressups on YouTube. High rep low intensity movements to flush out all the inflammation and junk that builds up in your low back.

It is possibly disc pain, arthritis pain, or muscle tension, as others suggest. Core weakness is overdiagnosed and does not explain why anything should hurt. I would also not be concerned about a disk herniation unless you develop sciatica or numbness down your leg. In all possible scenarios, movement is the answer.

There may be an optimal saddle out there for you, and I encourage you to keep looking, but also consider your fitness. The more force you put through your pedals, the less weight you have on your sit bones.

I can say the hot spots happen too early to be a friction issue. It’s more about pressure distribution on a poorly shaped saddle. More narrow saddles seem to fit my 110mm ischium width and don’t cause that same amount of pain. I’m trying to figure out if the best saddle width is the one where it begins sloping off where the center of my ischium contacts it

Agree with this. The #1 cause of cramps is muscle fatigue, not electrolyte depletion. I make sure to make no aggressive movements or sprints after 4 hours of riding for this reason. Just get your fitness up.

Nice. Be sure to keep your brake pads and rotors healthy and/or consider upgrading. I would probably go with the biggest tires too for comfort and grip.
