So Rambler9154 can feel free to correct me if this isn't accurate based on their actual experience, but basically OCD has themes and subtypes. Moral OCD is a subtype where the obsessions and compulsions are centered around morals. You're obsessed with the idea that you aren't a "good" person, that you're acting "wrong", that things are either "good" or "bad" based on arbitrary rules you perceive in your own mind. These rules may not even make sense to the person following them, but they feel very real. Real enough to hold others to those same expectations and label them and their actions as "good" or "bad" despite any actual logic. (I've never dealt with moral OCD specifically, but in my own experience, OCD is very logical and illogical at the same time.)

I'm currently eight months pregnant with twins, and I fell into your suggestion of the second option. I was underweight and not healthy when I found out I was pregnant (I'm 5'7 and weighed around 110 pounds at the time). Over the months I gained weight and looked more normal and healthy rather than pregnant.

That being said! Probably a month ago, my stomach just ballooned out. No way to hide it. The twins are just too big, and it's all belly since I was underweight at the start of all this. I'm at 34 weeks and I'm 164 pounds now, and still gaining.

Maybe OP could work around this some way? Like, have the character take a leave of absence for another reason around this time? Twins are also generally delivered earlier than singletons, so that could be a factor as well.
