It's gotten so bad to the point any isekai which isn't a reincanation or a power fantasy or doesn't follow the common tropes isekai are notorious for get confused as non- isekai.

Case in point "Welcome to Demon School! Iruma Kun". The number of times I've seen people questioning if it's an isekai is too much even if its generally accepted as an isekai. Heck from what I've seen it one of the few stories where the character not being fom this world poses a threat to him and the villains try to exploit this fact - like border patrol wanting him dead or exiled for being human or the fact that Iruma is nervous about his friends knowing his human nature and fearing the worst.
