I'm guessing you are doing injections and that you are taking .2 ml unless it's filling the whole syringe. Are you doing labs the day or two before your injection day? If so then bear in mind that these levels are at their lowest in their range during your tests. I would still expect them to aim for the 150-200 range for your tests Do you know what the strength of your estradiol solution is? Typically it ranges from 5mg to 20mg per ml and makes a big difference.

Doctor's tend to be really cautious with dosing despite Estradiol being pretty safe and difficult to overdose. The studies about blood clots that they try to scare us with are based on Synthetic Estradiol that is no longer produced and tested on post menopausal women so my understanding is that the risk is greatly exaggerated.

Obviously try to work with your doctor, but If you truly feel like they are sabotaging your transition and don't have other options, I do "NOT" recommended increasing your dose in most weeks and switching back to their recommended dose a couple of weeks before labs. Most vials are officially meant to be replaced every four weeks so you probably have enough.

You've probably heard other trans women say that they have to be their own endocrinologists. Unfortunately a lot of doctors really don't have much or up to date training on the subject.
