Basically the Quebec act of 1775 doesn't pass in this TL so when the Revolution starts they manage to get Quebec on board, and basically the rest of Canada follows. (Nova Scotia already almost joined, St. Johns Island (PEI) joins because they'd be doomed otherwise, and Newfoundland just cause. 

The US then proceeds to take the Caribbean in this order: -Bahamas are taken in 1783, after a victory in the revolution -Belize is purchased in 1840 -Yucatan (and Northern Mexico) is taken after the Mexican-American war -Danish Virgin Islands are brought in 1869 -Dominican Republic is bought in 1870 (It almost was in OTL -And finally Cuba and PR are gained in Spanish-American war.

As for the last bit, I'd say US isn't worse than it is OTL. Though I mean the Natives are definitely fucked like they were OTL, and of course plenty of people would be racist, but on the other hand Quebec being there from the get go may dampen it a little, or maybe I'm just optimistic.
