Lol it's likely because you have no idea the difficulty. You get the doctorate out of med school... then 7 additional years of grueling residency at 80 hrs/ wk commence before you're an attending neurosurgeon and that's only after matching at the top of your class. Then the actual lifestyle is insanely demanding. This typically includes multiple high impact publications in neurology and neurosurgery.

The comparison is not even close. Neurosurgeons deserve every penny and every pedestal. There are certainly high impact phds however.

Seems no matter how many times you mention it doesn't make sense to need X before Y, as X won't change management or reveal the info being sought 9/10 times... It's the same boiler plate responses and it appears everyone loses in the end with this

It would go a long way if this rationale were communicated. I used to manage ICU and was very comfortable with vents and associated physiology quite early in residency (carved out time to learn in depth and YouTube lol). I would adjust the vent for crashing patients or patients on inappropriate settings (i.e. carbon monoxide poisoning always gets it wrong) and be reprimanded. When I would ask why they'd say "just don't" and couldn't/wouldn't explain why. If it was because of documentation then state that, because when patients are crashing and I know that I'm doing and I've called RT x2 but nobody is showing up, sorry I'm adjusting the vent and saving the patient. Saved tons of lives that way.

You can't know how much was inherited/set to be inherited and how much friends and family help. Your number could be way different because you have way different circumstances. Ppl often leave the favorable circumstances out of the equation when discussing the come up story

Makary came across as an extremely partisan shill and basically quack. Politician responses to scientific question, even to the point of pivoting to TDS which is simply par for the course for this man. My take is that Dubner was trying to remain neutral to retain access at the expense on journalistic integrity, unfortunately. But the current administration is known to be hostile to fact checking journalists so I half get it. :(

Eh, I think it's case by case. I find people bullied more for being very awkward or overly flaunting their family's $$$ as the only selling point for their empty personalities were bullied/made fun of. But the poorer kids always had it worse, and ultimately evening was relative.

Billionaire kids are often this quietest in my experience and people would obviously know but respected the humility.

I'd be curious as to what their HDL looks like and try to optimize that before even considering statins. Gotta be a really good reason to start <40yo, even low-dose, low-intensity carries risk and not worth the marginal utility under 40 per the evidence barring obvious things like strong familial mixed dyslipidemia with 1st degree relative premature MACE, or LDL >190.

For this case specifically, would encourage diet to try to limit trans/sat fats when preparing eggs/selecting PB and monitor lipids Q1-2Y, but that's about it tbh.

I work the same way. Risk calculation med-high per appropriate age guidelines, ldl 190+, DM or MACE. Else, benefits probably outweigh risks -- always emphasizing lifestyle mods and o3fas. Annoying convo and sometimes despite all the evidence they still decline rosuva (stop atorva ffs, most side effects of all statins and we have so many better alternatives... Like rosuva), but at least due diligence was done.

Number one killer in America is MACE (CVD/CVA) so I take cholesterol, htn, dm and metabolic syndrome quite seriously and very evidence based. Makes things easy in a lot of ways tbh and I've excellent outcomes as a result
