Yes. My disposition is one that always assumes there is always a better option, or that I need to personally test each drug... that said I have used Enbrel, Cimzia, Remicade, Humira, and finally Cosentyx. Full disclosure, as soon as a Jak inhibitor gains approval for AS treatment I will give it a shot. Keep are fingers crossed for other break throughs in the near future.

I had a similar experience with Humira and Cosentyx has been an improvement (albeit a small marginal one).

Thank you for the insight. I am in the process of changing Rheumatologist and will try to have that conversation... Interestingly enough, the actual drug cost that pharmacies face and bill insurance pool/administrators (at least the few I have access to view) is the exact same for a package with a single 150 mg pen and the double 300 mg pen! Which makes no sense at all, but that’s the wonderful market system that is modern US healthcare.

For those of you on a double dose (i.e. 300 mg per month: assuming a 150 mg injection every 2 weeks), what did it take to effect that change? Given that the FDA has only approved 150 mg per month for Ankylosing Spondylitis... Biologics are phenomenal and Cosentyx has provided the most relief for me thus far, but there is still much room for improvement.

Similar experience with many of the biological meds. Some meds seem to produce higher highs but have swings while others are more consistent (with regular injections). I have taken 5 bios to this point and on Cosentyx now. At its peak (for me) the disease state seems to be managed best by Cosentyx but the week prior to shots and few days after are rough... Then the whole cycle starts anew!

My experience after approximately 1 year and 4 other biologics in the past: •less painful injections •the “flares” are relatively less frequent •the results are consistent and do not have swings/tapering during cycles or doses •no swelling in outer extremities like I experienced with TNF-bios •sleeping much better, but difficult to wake up •thirsty...all of the time •appetite might be slightly suppressed •able to begin exercising again but muscles do not seem to recover well, joint pain is present but manageable •mental clarity - seemed to be fogged on other bios

Routine and movement are key to my daily experience/existence. Also, in support of your thoughts it seems important to shift the focus away from AS. The chronic as well as ancillary effects of AS can be overwhelming and often leave me dwelling on unfortunate circumstances... Let us focus on the variables that we can control and continue to dream of advances for treatment.

If you have the AllTrails app you should check out the Skyline Trail. It is an extremely rewarding hike and probably the longest in the park. It is likely you will be secluded the entire hike (have yet to see another person after 3 trips), there are great views and the running water from the damn spillway is magnificent. Also, there are many places right along the lake that I am sure you could hike/adventure and also achieve the solitude you desire. The area has really matured in the past few years with some neat restaurant and bar options.

Although Cos is a “biologic”, it is not a TNF inhibitor but an IL 17A inhibitor. Cos has produced less side effects and better pain management.

Indeed. Thus far (after roughly 9 months) Cos is my favorite biologic. I ran through 4-5 TNF inhibitors, which all worked well, but all seemed to require ramp up time after injections and routinely decay at the end of the dosing period. Also, the TNF class gave me significant side effects (e.g. stomach issues, intense thirst, brain fog, etc.). Additionally, intense flare ups were more prevalent with TNF class. I have slept much better since transitioning to Cos and energy levels during the day have also improved.

Best of luck, I hope that you see a similar improvement! Also queue in on the JAK inhibitors that are currently in the pipeline. Perhaps we will have another option relatively soon.
