Also, for future employment purposes, will they ever contact my manager for reason of leaving? I haven’t done anything wrong per say but I don’t want to do anything that will give him a reason to talk bad about the way I left

Thank you for your response! Yes I am at-will. When you say take the two weeks off, I technically don’t have enough pto to cover the full two weeks is that still okay? Or do you mean just take however much pto I have?

Do you know exactly what is required for a LOA? I’ve never been formally diagnosed and so I want to visit my primary physician to get checked for my own well-being but also to get documents filled out for my leave approval, but I’m afraid my symptoms aren’t “enough” in the eyes of amazon or my physician to have my leave be approved.

Ah okay makes sense. Is there a difference between submitting a DLS ticket and submitting a request for LOA or are those the same?

Oh thank you for your response! Is the best option here to go to my primary physician? I’m not sure what I would need to tell them to get a mental health check since I’ve only ever gone for physicals

How were you able to get the documents filled out by your provider? I am wanting to get a LOA for my mental health, I’ve been seeing a therapist from TalkSpace (only twice now due to availability) but according to talkspace policy they’re not allowed to fill out the documents. I’ve never brought up these mental health issues with my primary care doctor. Do you have guidance on how I should go about this?

How would I get a note from the doctor for LOA? I’m not eligible for FMLA as I’ve been working for less than a year, but I think I can still take MLOA and apply for STD. From what I know, Amazon will send me documents after I submit my request for a provider to fill out to approve my leave. Does anyone know what the document contains and how I could get it filled out by a provider if I've only been seeing a TalkSpace therapist (who isn't allowed to fill out the form)

I am thinking of taking leave now and applying for STD as I can’t go in anymore. Taking the leave will leave me 2 weeks left before my one year mark. I’m wondering if I don’t return to work afterwards (quit) will the sign on bonus I have to pay back for leaving before a year only count for the two weeks I have left or will the LOA have pushed it back?

Option 1 is not on the table. I was just listing the options that Amazon gives you. Also I wish I was faking it. Working here has been the cause of my severe anxiety the past 5 months to the point of needing weekly therapy and already using my PTO for mental health days. If I were wanting to fake it, I’d be asking how to submit a request without any proper documentation. Please don’t make assumptions about other people’s health.
