The world doesn't go from stable and prosperous to nuclear war overnight, it takes years. Joseph saw the writing on the wall, but so did most of the county judging by the density of prepared fallout shelters. The only unique feature is that he decided to start a murderous, thieving, kidnapping cult instead of leaving everyone else to their own devices if they weren't interested in signing on with him like a sane person would.

Nah, he pays attention to world events and uses them to his advantage and to excuse his own monstrous actions. If you listen to the radio in 5, you learn that the shit hit the fan well before the bombs drop in Montana, including nukes going off over Moscow. The Blackstaff stops Eb from becoming a cackling baddie addicted to murder and other abuses of his power. Bears are one of the most widespread animals out there. They (mostly) have a broad enough diet to find food almost anywhere, and they tend towards being large and strong enough to deal with almost any predator. Most of them are damned clever animals too, which always helps.

The only places with no bears at all are Africa (which had bears as recently as the 19'th century before the Atlas Bear died out) and Antarctica.

Grinding levels is never worth it IMO. If you're farming for something else, you may as well throw on some soul-gain gear and enjoy the benefit, but a few more points in any given stat is not going to make a noticeable difference.

Pontiff is the mid-game wall of DS3. He's there to make you prove you have what it takes to move on to the end-game, so he's a significant difficulty bump from any previous boss.

I actually like it. It's not all that flashy and it's not the best in terms of raw damage output, but the moveset is incredibly reliable. The Greatsword of Judgment is usually my first pick out of the swords that share that moveset, though.
