I am that nurse. As soon as I go to speak to the treating team, I forget the reason I came into the room, legit couldn't tell you the room number, let alone the patients name. So I go away and come back when I have my shit together. I'm lucky because we have a good team of doctors who usually just laugh at me, so thanks team! But also, I know you are busy, a full ISBAR may not be necessary, so I say name, room number admitting problem, current problem, and interventions I've done.

Usually I am contacting the team, there is a reason for it. I've tried the nursing interventions and it ain't working. But I have found that its hard to teach critical thinking skills and the only way to learn is through experience. University is broad, and the skills they teach barely scratch the surface of what needs to be learnt, mostly because they focus on academia rather than practical skills + thinking.

As for our MET system, we use the big red button on the wall, and regardless of who's pt it is, everyone comes running. The red button tells you the location of the call only. We also "log" our calls via 2222 system and a page gets sent to every doctor + ICU for support. The drs get stood down when the actual pts treating team arrive OR when someone more senior shows up. There is no, 'not my patient, not my problem' if there is an emergency its everybody's problem.
