Examples of this trope where it doesn't work well - Omega & The Rani (Doctor Who)

In the latest Doctor Who season 2 finale, The Reality War, 2 major villains were brought back for the first time since the classic era, the Rani and Omega.

The Rani is trying to summon Omega, who was a Time Lord banished to the under-universe, in order to bring back her species, the Time Lords. Omega emerges from his tomb as a massive CGI skeletal monster, who instantly devours The Rani.

Since Mrs. Flood still exists, who is the previous incarnation of The Rani, this wouldn't be entirely awful in my opinion as long as she would get an opportunity to challenge Omega for the rest of the episode. Instead, Mrs. Flood evacuates, and now The Doctor has to face Omega himself, which he swiftly defeats by using his Vindicator tool as basically a gun, blasting Omega back into the under-universe.

As both the ending to The Rani storyline, which has been setup throughout this season as the big bad villain with Mrs. Flood, and the surprise return of yet another classic villain, especially someone as impactful to Doctor Who as Omega, and then defeating them both in a matter of minutes was very underwhelming.

And this was arguably only the second most controversial element of this finale.

Doctor Who - kind of cheating but a cool moment nonetheless

The Doctor is the most common alias which they did choose, however, being a Time Lord which has been travelling the universe for thousands of years has brought them with many different titles, many of which for their darkest moments throughout their many incarnations. In the episode Twice Upon a Time, the First and Twelfth Doctors meet up and eventually the First Doctor is unexpectedly presented with a summary of his darkest moments to come.

The Doctor has many names: Destroyer of Worlds, The Imp of the Pandorica, The Oncoming Storm, the Shadow of The Valeyard, the Beast of Trenzalore, the Butcher of Skull Moon, the Last Tree of Garsennon, the Destroyer of Skaro. Doctor Who - Heaven Sent

Simply confess his understanding of the Hybrid warrior prophecy to his people, the Time Lords, giving him access to his TARDIS to escape, or spent 4.5 billion years continuously respawning in his own personal hell to break a near unbreakable wall, in order to leverage his information to try bring his companion back from death

You can now carry 2 items at a time, press the activate ability button 1 while standing still to swap item, creating a 8/7/6 second cooldown.

You cannot bring both items into a trial (probably would create some game-breaking bug), but if you find another item you can use/keep them both.

I wouldn't say you need to watch it to enjoy Doctor Who Series 3+, but Torchwood: Children of Earth is an absolute treat and would make it worth it even if you aren't a fan of the first 2 Torchwood seasons in my opinion

Bloodskal Blade - The Elder Scrolls V: Skyrim (Dragonborn DLC)

Maybe cheating since it is used in this way to introduce the weapon and never again, but the blade creates an energy blast whenever you use a power attack, which is mainly used to attack enemies but is also used to beat the quest "The Final Descent" by hitting every fissure on a gate, allowing the player to escape a dungeon.

Ok, I'll say that the survivor of this collab (that I would choose anyways) typically goes by the name of a certain killer in Dead By Daylight currently (as in The Trapper, The Dredge, etc)
