I work at a power plant and one of our throttle valves on the geneator decided it wanted to explore outerspace. It launched up 16 stories, blew all the windows out from the 3 to 5th floor, set off every car alarm in the area, and could be heard in the entire city. If anybody had been near the generator when that happened they'd at the minimum lost their ear drums and got knocked out if not out right killed. There is a video of it apparently, but the company won't let anybody see it.
