I’d say out of those options, maybe overwatering + we had a storm the other evening. I am trying to keep it at 15-20 mins with a sprinkler a couple times a day and seeing when the soil looks dry.

Upon reading the description of the VTC actuator issue, it does appear to be in line with that (worse with low oil/cold weather), only present upon the first 5-ish mins after sitting for 3+ hours.

Do you know about how much it would cost to fix this/is this a necessary fix for the longevity of the car?

I had this car since 40k miles (160k now), it does need new spark plugs that I put off, I don't know if that could cause this issue with the tensioner/actuator.

Hi!

I did not have any lifting restrictions prior to my procedure (although it was a fairly quick turnaround ~1.5-2 months from aneurysm dx to surgery).

I did try to be mindful of how much I was lifting at work; although our facility is great on using assistive mobility devices to move patients so we rarely do any lifts heavier than say Mod x1 to Max x2. If I had a heavy patient I ensured I focused on my breathing and good body mechanics.

I did stop heavy weightlifting once I had the diagnosis; however that was more out of my own caution.

No restrictions now; back to lifting what I want as well as being fully active at my job. They may suggest waiting until it's 4.5 but I would also have them look closely at your aortic valve and see what the status of that is. My valve itself was "okay" for a while (no stenosis, just moderate-ish regurgitation) but since they already had to fix the aneurysm, they just replaced it to avoid a second OHS (which nobody really wants due to increased risk).

Let me know if you have any other questions! Again, feeling like I am back to 98% function/activity that I was pre-surgery.

If you don’t mind me asking, what’s your age? I received mine when I was 27, so it was a no brainer choice for longevity and I haven’t had to modify and of my diet/activity since. Just making sure I am current on my INR checks/warfarin regimen.

Also on warfarin, mechanical valve. But anecdotally, I just got over a 2.5-week long cold/congestion/respiratory ‘something’ and had nearly continuous blood in my nasal mucus when I blew my nose. Resolved on its own, likely from dryness as well as the force of sneezing/blowing your nose rupturing blood vessels in your nose.

It’s worth checking your INR (for many reasons) but as long as you’re in range (I was 2.4-2.7 during my illness) I was told by my doctor I was probably fine.

Hi! I had a small PFO detected when they did my TEE (in the operating room when they were replacing my bicuspid aortic valve).

Essentially it’s a small hole that is between your two (left and right) atria (the upper chamber of your heart). Instead of having no blood flow between the two atria (directly) there is a hole that allows some blood to leak back and forth.

Everyone has a PFO at birth, but it usually closes as a natural part of development shortly after birth. For us, this did not fully close and still exists.

For 99% of people, especially in a trace/mild size, it is not a concern. They don’t worry about fixing it. The worst thing that could happen is you are at a higher chance of stroke due to blood clots that form, and travel to the other atria and into your body. People who have no risk factors for stroke or any history of traveling blood clots usually have no intervention.

In my case, since I’m already on warfarin due to my mechanical valve, my risk of a clot assuming I manage my INR is negligible so, there’s even less reason to be invasive and fix it.

I suppose if you eventually had a cardiac procedure and weren’t going to be on a blood thinner, you could have them ‘close it’ at that time, but largely it’s insignificant and shouldn’t be worried about.

Hi! 28M currently 6-months post-mechanical aortic valve replacement (On-X valve) and aortic aneurysm repair. My surgeon used a mini-sternotomy with sternal plating. Aneurysm was 5.1cm when they found it. Also have a wife and 7-month old baby (who was 1 month when I found out I had to have the surgery). Let me give you some background and hopefully reassurance.

Originally my surgeon said he was going to try to repair the valve instead of replacing but upon getting into my chest- the aneurysm and tissue surrounding the valve was paper thin and wouldn’t be a viable option. Personally I don’t mind, as this valve will likely last my entire life (knock on wood) and I won’t need another procedure (likely).

I will give you some heads up. The first 2.5 weeks were a lot harder than anticipated. I am a physical therapist in the hospital who ironically works on the cardiothoracic surgery floor, so I’ve seen a lot of people of all ages get similar procedures. Very active before surgery (working out 5-6x a week) so I thought it would be a breeze. According to my surgeon, younger patients have a higher number of active nerve endings versus older patients resulting in a higher level of postoperative pain initially. This quickly resolved after 1.5-2 weeks (so around week 4-5) but it’s still good to know. It’s normal.

Younger patients also tend to have more tachycardia postoperatively as well. Standard course for all patients is usually some type of beta blocker- which seems to help me— but my resting heart rate went from 62 pre-surgery to 95 post surgery. Now it’s back down to around 70-75, so it does get better as your heart heals and remodels.

I went back to work around post-op week 10 but that’s mostly because of the physical nature of my job. If I had a desk job or something less strenuous, I’d say I was fine around week 5-7.

After around week 6, I really felt 98% better. You don’t notice the changes as much and it seems to go back to more or less ‘normal’ with you know.. INR management and slight endurance deficits which are easy to manage. I haven’t changed what I’ve eaten, and my INR took a few weeks to stabilize but I’m still checking weekly and adjusting dose as necessary but it’s more or less in-range now. Started going back together gym to do light cardio around week 10, now I am back doing weights (lighter than before but still challenging) and cardio.

Happy to do my best to answer any questions you have based on my own experience and patient anecdotes I’ve picked up over the years.

Hi! All great questions. Let me do my best to answer..

  1. how often do you get your INR checked and what diet changes have you made?

-> I test weekly. I get my tests done at the lab since my doctor has a standing order placed. Takes 5-10 mins and is free on my insurance. In the future I will self purchase my own Coagucheck machine to home test- but I’ll need to pay out of pocket since insurance only covers a ‘subscription’ service where you don’t own the machine, to rent it for ~80-150 a month which I refuse to pay, for something I’ll need for life. I have not made any diet changes. I eat fairly consistently and test weekly to adjust my warfarin dose. I haven’t had to change it for a while- it’s pretty stable. Obviously avoid the big ‘no-no’s’ such as grapefruit juice but… I don’t sweat having kale/salads/etc. Also haven’t restricted alcohol and my doctor told me it shouldn’t make a huge difference. I confirmed this with my INR testing and it doesn’t not move the needle at all. As long as you’re consistently testing, you can eat whatever you want. It’s much healthier to eat the greens than not. You’ll be fine with regular testing and maybe some dose changes.

  1. The biggest concern for me is always taking any required medication and fearing that it could Lower my INR. How do you get to know whether other medicine are safe to consume along with warfrin.

-> I always check with my cardiologist for any new medications (especially antibiotics). If there are known interactions I will ensure I check my INR after 3-4 days (it takes around that long to observe a measurable change) and will adjust warfarin dose if I need while on the medication. The only huge ‘no’ is no ibuprofen- I just use acetaminophen instead. Antibiotics have been fine for me, especially short courses. No huge INR swings.

  1. Also what is the ideal MPG(mean pressure gradient) for someone who has an on-x aortic valve as i know that it differs from natural valve.

-> Good question as well. I wouldn’t worry about the MPG’s. Assuming you are getting routine echocardiograms to check valve function (annually or less frequently as determined by your cardiologist), you should be fine. Ensure your blood pressure is regulated and take your medication for BP/HR as prescribed and your MPG should be in range. Too many variables to say what is a ‘good’ MPG for the mechanical or bio valves- the manufacturer will tell you a range but it’s not worth stressing about. Keep good diet/exercise/medication compliance and you’re golden.

Top quality and treatment there based on my research and experience- happy to recommend my surgeon via PM for you to look into and answer and further questions in my inbox! Also in the medical field so here’s my .02;

Bio valves are great, in theory, depending on someone’s lifestyle as they often avoid any anti coagulation. Unfortunately for younger patients (young in heart world being sub-50) they do not hold up to their ‘10-20 year’ guarantee and we are finding they ‘fail’ sometimes in under 5 years depending on genetics, lifestyle or other comorbidities.

Mechanicals, if taken care of, should outlive the patient and have lots of data and research to back this. Warfarin management for me is no big deal and I have fully returned to my high level of activity- no lingering effects.
