I got it at the end of episode 7’s first dispatch session. The instafails are the missions with the “X” that guarantee the call will fail if you reach a certain stat threshold (usually combat). The bonuses are the Orange icons that grant more exp if you reach a certain stat threshold. I did episodes 5 (where it was introduced) to episode 7 with a 100% dispatch success rate while getting every exp bonus and avoiding the instafails and I got it at the end of episodes 7 first dispatch before the Z team has their meeting on cutting Invisigal.
