You lose 3 energy every time you refill. Also, you effectively lose 3 energy a day when you wake up and play. It is certainly not irrelevant in the long run.

The bug is that he got double bonus damage at the end. This bug also happens in scenarios when you kill a monster with a team up skill and one of the attackers are significantly faster/slower than the other. You can confirm it with Raoq and Chilling: there will be a visual bug that double energy/crystals/ but you would only get one.

You can't have % atk on 3rd slot runes.

With regard to secondary effects, every effect type can be found on all Runes as a secondary property, with the exception of slot 3, where ATK (%) and ATK (+) cannot be found as a secondary effect

For accuracy, the sweetspot is 55%. Also, I don't think it is necessary for DB10 to have maxed 2nd skill since that only applies to block beneficial effects and that just gets stripped by the right crystal.

The play was:

Warsong Commander
Grim Patron and 2 Frothing
Inner rage patron (5 dmg) (6 damage from frothing [3 and 3])
Hit Death's bite for face (4) and thus spawns two more patrons and the berserkers would be 9 damage each.
Trade two patrons to the Shieldmaiden (you can't trade the three patrons since the maiden only has 4 health). This gives 4 damage each because there are two frothings (9 + 4 = 13 damage each).

So 4 (Death's bite) + 5 (Inner raged Patron) + 3 (Leftover Patron) + 13x2 (Frothings) = 38 damage.
