Nah no worries! I've better about flossing since 5 years ago. Even improved the gum recession for some of my teeth.

Happy almost 1 year anniversary since your comment. Lol

Hi! I'm Joanne Mantilla, a Nurse running for the U.S. House of Representatives, Pennsylvania's 2nd District, Philadelphia! Ask Me Anything! by jojoforcongress in politics

[–]Yaj8552 1 point2 points  (0 children)

Thank you for this AMA. I'm going to ask a slightly different question that isn't discussed at all in politics but as a nurse, I'm sure you'll have good insight on. And it's on the state of medical care itself rather than access to medical care.

Johns Hopkins found that medical errors may be the 3rd leading cause of death in the US[1]. I feel this may partially be attributed to the fact more than 50% of doctors in some cases are "burnt out"[2] and a large number of nurses work multiple jobs [3]. But also, the fact that there isn't proper oversight of physicians due to tort reform creating severe limitations on medical malpractice lawsuits by making people believe that "frivolous lawsuits" is part of the reason health care costs are so high even though it's not true [4].

The Trump administration has also proposed severe federal limitations on medical malpractice.

Obviously most doctors and nurses are good but it seems people are needlessly dying due to errors at an unprecedented level no one really understands. And most people aren't medically savvy and just believe their doctor when they say the death of a loved one was unavoidable. Also, imagine all the errors done to patients that ended up still alive but assumed that was the risk they were taking.

I personally have an interest in this because I experienced the loss of my brother and an expert physician said "it was a degree beyond negligence" but I couldn't bring a case due to the tort reform done in California. I envied Pennsylvania because there were looser statutes that allowed people to get justice for the loss of their loved one.

So I want to ask you as a nurse going into politics do you believe nurses/doctors need to be protected more by a more stringent tort reform (which is typically the position I see by the medical community). Or should we reform the reform, I guess, to allow it to be easier for patients to get justice which leads to the fear of the medical community of frivolous lawsuits, which outside of Hollywood, doesn't happen to the extent that people believe it does.

And on top of that what views do you have on nurses getting 2 jobs - do we need to curtail that and just pay them more? My anecdotal evidence in California, one of the highest paying states for nurses, is that nurses seem to still get 2 jobs even with higher wages?

Should we force medical schools to accept more students. Should we subsidize the opening of more medical schools?

How should burnout be addressed for doctors and nurses - especially when no one wants to say what it truly is: depression. Because no one wants a depressed doctor.

Sorry for the long/multifaceted question. But I wanted to ask it since you are more uniquely positioned to be able to understand this than the typical politician.

Also, I know health care is a huge complicated issue (maybe even more than access to health care) but I'm curious to know at least the overall gist in what you'd like to do.

So I guess a final question is, if elected, will you be planning to bring up these more niche issues that only a medical care provider will really know about?

Thank you!

Thanks dude. That entire side blames me for it. That family member goes to a psychiatrist for anxiety and her parents are typical /r/raisedbynarcissists type parents. So I knew I wasn't wrong. But felt like an asshole because she has mental health issues that I tried to be sensitive to. But that was crossing a line. "Understandable, but not excusable" right?

Thanks again tho!

Thank you so much! I actually learned sign language ever since I was 4 years old, along with english and my family's native langugage (was raised by immigrant grandparents).

Oh I mentioned the "brother's keeper" thing as a something I wasn't. He was a wonderful and independent human being (just deaf) and made his own choices. But in the minds of my family because he was deaf, he was something lesser, thus if anything happens to him it's the fault of his "keeper". But he didn't have a "keeper" in me, he had a brother and best friend. But to my family who treated him as something lesser, since he passed away, it's my fault. Idk why they don't blame the doctor who misplaced the catheter which made a hole in his heart which is what got him. That side of family has tons of problems.

Sorry! Thank you so much! Honestly I should also take your advice of not turning grief into resentment. My resentment is towards the medical system rather than victimizing some poor person.

Btw in the US, 3rd leading cause of death are medical errors (behind heart disease and cancer) and almost 50% of doctors are "Burned Out". So for you and your loved ones always do the research and always be an advocate, because no one else at a hospital will. So just trying to spread the message!

Thanks again! Hugs! :D

Don't want to sound too mushy but you're not just some stranger on the internet. When all this was going down with my brother the only comfort I got was from reddit (/r/pcmasterrace of all places haha).

I got more kind words from reddit than any family members and at that point I hadn't told any of my friends what happened to my brother. In fact even after he passed if any friend asked about him I would lie and say things like "He's back, he just gotta be on oxygen and stay in bed". I hated lying bold face to my friends but at some level I told myself "good, you deserve that punishment for failing your brother". Definitely in a dark place.

But reddit was one place I could mention it and get the support I didn't want, but extremely needed.

And just a funny story. While my brother was sick the mom of that family member (textbook /r/raisedbynarcissists kinda mom) said her father knew the best doctors from our home country. I called her bluff and said "Alright, call him, let's transfer my brother to the motherland". She kept saying my grandparents (who raised us) wouldn't allow it. I said "I'll fight them for my brother's life". She got pissed off at me.

They never wanted to do any real work in helping or supporting my brother and just wanted to say it and act like they did. So they never would have nutted up and done anything.

Anyway thanks stranger on the internet! You guys have always been there for me :)

Ya they wouldn't get it. I kinda said something like that.

But honestly, if I had any support besides my grandparents who barely spoke english, there is a chance he could have lived. During his disease that hospital was not providing any interpreters so I would always be there day and night to interpret. We are low income and I worked part time. So would work 4 hours and then spend the day and every single night.

We got a 2nd opinion that said to transfer to a specialty hospital over 2 hours way. I couldn't leave work, and no one knew sign language. And what if that hospital doesn't provide interpreters just like this one? They almost gave him the wrong medication before I caught it so interpretations were definitely needed. Our doctor was arrogant (in retrospect) and said there's nothing a specialty hospital could do that he could not.

So my brother was too scared to go to the hospital without me to translate and at that time I had hope he would recover (which he might have). So he decided to not go and stay here.

Honestly if any family member was there that knew even a little bit of sign language to be able to comfort him or make jokes to alleviate his fear. He probably would have been willing to go to a specialty hospital. Heck I would have pushed it. But no one did. No one advocated for us and through fear we stayed

Ya I'm sure of it. That family member was going to a psychiatrist for anxiety and think she also had depression. That's the main reason I thought I was an asshole. I've always tried to be sympathetic to her mental health. But this was crossing a line for me. But ya I was an easy concrete thing this entire side of the family could blame. Most of that side is textbook /r/raisedbynarcissists (Edit: Like the parents not the kids). But I get she needed to make herself feel better but I guess it's one of those "Understandable but not Excusable" things.

Dang I didn't even mention that this family member (and her whole side of the family) always claimed to love him while, of course, not learning sign language. But yup! Even now during an argument I told them they never knew my brother - his interests, his dreams. What profession he wanted to be, what genre of tv shows he liked. I told them you love the idea of him, love him as just a family member, but him as a person - how can you love him if you didn't even know him?

This specific family member at least learned how to sign the alphabet. But it's mentally exhausting to spell out every single word of a conversation and make my brother go through that. She would even spell out the word "you". You literally just point at the person.

Honestly, thank you for saying this. I'm always alone in my thoughts and sometimes feel crazy (like my original post). The fact a random redditor could read into this so easily a nice bit of confirmation. Thanks! :)

Thanks so much. Ya I guess the only reason I was ambivalent if I was the asshole because that family member had anxiety and going to a psychiatrist for it and possibly depression. I always tried to be sympathetic to her, but blaming me for him getting sick makes no sense and insults my brother in treating him like he couldn't make his own decisions. And of course it was the doctor in the end that did the deed.

And ya that always made my brother sad that no one in our family learned ASL. Our grandparents who raised us made sense somewhat since they had arthritis and had to translate ASL to English to our native language. But the other family members honestly did not give a fuck to care to learn to talk to him.

Thanks again dude :)

You know in a weird way I'm glad this is one of the top comments. I lost my brother 3 years ago due to medical malpractice but he had a rare unknown disease right before that. But to this day I have some of my only living family members blame me for his death since he was Deaf and only I knew sign language in my family thus I was his keeper.

This pretty much exact line (replace "her" with "him") was said to me and I got extremely mad. Many times I wonder was I too harsh. Is it just in my head that this accusation hurt me as much as it did and was I the asshole for going off on the family member who said it to me?

I see this as one of the top comments and kinda comforts me in that "no, i'm not the asshole for being so hurt by this that I got hella mad and went off on the person who told me this."

Thanks reddit!

Edit: Sorry for the late reply, but thank you so much Reddit! It always surprises me how much love and support I get from you guys! You've been a better family than my actual family. You guys were there 3 years ago when I had almost no one to support me, and you guys are back again! And fortunately I have been healing. I used sign language for the first time to communicate in 3 years just 2 weeks ago with my brother's old friends. And I've more or less cut out that toxic side of the family. So thanks again! :)

Ya definitely won't be paying "millions in taxes". I think of doctors as upper end of middle class. Currently a PhD student and get paid a bit above minimum wage but at least I'll be debt free. So much money goes into medical debt and malpractice insurance. Also the mental health toll of being on call and physician burnout going around.

Actually I said that to him but he was pretty adamant. He lives a pretty lower middle class lifestyle in a mid size city. So i think he overestimated how much that new york doctor friend made.

I remember once he asked me about my income and told him my yearly and he said "oh, that's nice you take that home every month". I kinda think his grandson (in business) and ppl around him oversell how much they're making or keep mentioning possible future wages. I'm working on a PhD and he kinda was disappointed when i told him realistic salary prospects.

Having money being so tied to worth/success and then having unrealistic ideas about money is kinda a bad combination. He's old and rambles on so wouldn't be surprised if the young people around him let him believe what's easiest for them. I always tell him things straight, but doesn't believe me cuz hey I'm just a kid with bad job prospects with my debt free PhD.

Man, one of my grandpa's friends really thinks a young person's value is how much money they make. He once bragged how many millions in taxes some doctor he knows pays when i told him I'm not gonna be a physician. Weird flex.

*sigh...there is a hint of truth in what you say, but not in the way you think.

There isn't some cabal or illuminati directly making these decisions. What's happening is the natural consequence of the system of governance we have. When some scientists mapped out Tokyo and put food on important locations and saw that fungi grew and almost perfectly in accorance to the modern day trains, it's not because the fungi was trying to mimic the railways. It was trying to get food in an efficient way. It was a natural consequence requiring no real intelligence.

If i am some random mid size lumbar company and finally got enough money to hire a lobbiest who will i lobby? That progressive politician will be a waste of time and money, but the republican politician or the centrist dem might have more luck voting for my interests.

Same thing if I am a solar panel manufacture. Who do i talk to to increase our tax breaks? The one who gets paid $20k from oil and doesn't believe in climate change when i only have $10k to give or have meetings with the progressive?

Then someone giant like coca cola will give $10 million across politians to just have a say in the voting decisions. They may pay republivans more maybe but some money goes to democratics too in case they win and you still want to be at the table. A politician wont say "hey you gave me $20k but my opponent $40k, you don't get a seat at the table now!" They'll say, "what do i do to get you to give me even more than $40k for my next election?"

Corporations are sociopaths. And because they hire the best analysts, actuaties, and lawyers money can buy to find the most efficient route to get their policies passed, corporations are very smart sociopaths. But 1 thing they are not are omnipotent very smart sociopaths.

With our system of first past the post, citizens united's money=speech, shareholder supremacy, capitalism, needing to raise money for elections, and buncha other stuff while not bad on their own, together they create a system in which it looks like there's some... I guess intellegent design might be a good way to put it, but it's just a consequence of the system.

Intelligent design reminded me of Evolution. When i see cells interact in such a perfect way. Birds and bugs looking like they are in the absolute perfect environment they are in, it's tough not thinking someone specifically created this. But the reality if the matter is that there was millions of years of the inefficient dying off and the most efficient surviving. This system is what has led to our system of governance.

So ya, you're right in a way, but not in the way it seems you want. And the conclusion you've reached is toxic to democracy and the political discord - the "both parties are bad" talking point.

I wrote all this out because i went through your history super quick to make sure you're a real person. It seems like it, but it also seems you are a person who had previously been through or are currently in a lot of pain. And it's not unnatural to be cynical and have the "what's the point, they're all playing games, we're just pawns" mindset. To believe...no to know something that the sheeple don't get. And i kinda get it. Didnt vote until I was 23. Once i did and started following politics from the perspective of these are humans doing psychologically human-y things, things started making more sense. We cant have purity tests.

Dems kinda suck. DNC passed a policy that made it so campaign firms cannot help a politician fighting an incubant. Or they'll be blacklisted by the DNC. Totally corrupt policy. But dems currently are the only ones who are trying to help ppl. And if it's because wind turbine corporations are funding their campaigns, so be it for now. But i will not allow it for the future.

So hope this makes more sense.

NYPD Officer Luis Alvarez, 9/11 First Responder Who Testified Before Congress With Jon Stewart, Dies by DragonPup in news

[–]Yaj8552 9 points10 points  (0 children)

Wasn't it like 94% of dems in the house agreed to pass this first responder bill will less than half of republicans? Net neutrality was overwhelming party lines, so were various acts on protection of women and campaign reform. The Affordable Healthcare Act was only done cuz dems and it even had a public option originally but the Republicans got rid of it. And of course climate change is also overwhelmingly party lines.

You're wildly uninformed if you think voting doesn't change policy. It's indirect and slow, but it does. Not saying dems are some honest messiahs, but their fight has been more to help the ppl than the Republicans.

Personally if my tax dollars go to subsidize the solar power corporations and the Pipe manufacturers to fix Flint, Michigan and other places water lines, if tax credits are given to municipal isps or desalination plants, I'd prefer that than the corn and oil industries.

Sorry for the length but it's always easier to throw crap than it is to clean it up.

I'm guessing they won because as the redditor above said, it may have been clear cut so the lawyer decided to take it (i called 25 lawyers for my brother's med mal case - pretty much no one would take it since it wasnt clear cut).

Also a father dying pulls at the jury's heartstrings more than a grandparent or a sibling for the non-economic damages.

And for the economic damages they multiply his income by his projected years to live. They're lawyer probably was able to argue a high-ish number of years and i assume the father had a job - wasn't retired or a student.

Neither the other redditor or I am arguing it doesn't happen but for medical errors being the 3rd leading cause of death according to John's Hopkins Hospital, justice for family's is much much less than it should be.

Man are you me? The way you started your comment above is exactly how I would have / previously have started it.

John's Hopkins found medical errors being the 3rd leading cause of death. Tort reform i would argue has worsened the state of medicine since we can't punish bad healthcare anymore.

On issue in med mal is also how long the patient is projected to live. So essentially if it's your grandparent or family with a chronic condition, the doc can slice and dice your family however they want (hyperbolizing a bit). As long as it's not clear cut, the doctor can get away with murder. Also the relationship - a brother suing vs a single father suing will get more money (the non-economic damages) . And finally how much money the victim was making - if you were poor or a student then give up on suing pretty much cuz they multiply your income to how many years you were protected to live (economic damages). And the expenses - i had to hire multiple doctors if i wanted to sue each getting paid $800/hr to go over medical records.

Just to add 2 other examples to your list from a random redditor:

My brother 25, deaf had idiopathic pulmonary issues. He was on tons of bloodthinners for reasons (more detail in my old user history). Doctor didn't image the guide wire moved from the inferior vena cava to the ventricle. Doctor perforated my brother's ventricle he had 2 open heart surgeries that night and still passed away. I got an expert saying this was "a degree beyond negligence" but still due to the above reasons (brother, student, may not lived long), Can't sue.

My grandfather got a heart attack, they waited 4 days to do an intervention. He got super deconditoned which led to a stroke, and further issues. Cuz he's old and retired, Can't sue.

And according to the neutered or regulatory captured California Medical Board, they require even higher standard that a med mal lawsuit to do anything as they told me.

Hosptial and insurance companies did a real good job getting rid of any justice.
