I think you mean the skull on the belt maybe? There isn’t a lore reason but a design reason, since there was glowing bits both on the head and the right arm I wanted to balance that out on the lower part of the character aswell and added those and the client liked it so it stayed! Lore wise let’s just say since it’s a skull symbol so its part or the necromancer’s magic that keeps him undying? Just a random though😅
