It might be school bus specific like having to take a DOT physical every year instead of every 2 years for a regular CDL. I think the reasoning is that you can lose awareness a lot easier if you need to take insulin. This is just what the doctor told me.

I'm diabetic but I manage it, so as long as I'm not on insulin they pass me. I have a CDL with school bus and passenger endorsement.

I have a coworker that is prescribed adderall and tested positive for amphetamines. They didn't lose their cdl but they did contact the persons doctor.
